Anaesthesia and juvenile hyaline fibromatosis

Abstract: Juvenile hyaline fibromatosis is a rare autosomal recessive disease characterized by large cutaneous nodules, especially around the head and neck and often involving the lips. The effects become increasingly severe with age and also include joint contractures, gingival hypertrophy and osteolytic lesions. We describe the anaesthetic management of two sisters with this disease. Safe maintenance of a patent airway is the principal anaesthetic challenge.

“…Only cases of surgical treatment of labial and gingival hypertrophy in juvenile systemic hyalinosis have been reported. 8,14 In the present patient, hypertrophy of the upper and lower lips seriously interfered with routine oral hygiene, food intake, speech, and physical aspect. Gingival overgrowths fully covered the tooth crowns, leading to deep caries and later to pulp necrosis.…”

“…Other treatments for symptoms of joint involvement include cortisone, D-penicillamine and capsulotomy. Due to accumulation of abnormal materials in the soft tissues, severe airway problems can occur which may lead to anaesthetic complications 9. Death may occur due to pneumonia and respiratory failure due to peribronchial infiltrate and fibrous visceral pleural thickening.…”

“…TARTIŞMA JHF, özellikle artan yaşla parelel olarak boyun hareketlerinde kısıtlılık, temporomandibular eklemde kontraktür, dil ve trakeada hyalin birikimi, psödo hurler yüz deformitesi gibi ağız açıklığını sınırlayan nedenlerle hava yolu yönetiminin zor olabileceği belirtilen bir hastalıktır. (1)(2)(3)(4)(5)(6)(7)(8)(9) JHF'de anestezi yönetimi ile ilgili az sayıda olgu sunumu mevcuttur. Yapılan literatür taramasının sonuçları Tablo I'de özetlenmiştir.…”
