Analgetika med refusjon ved kroniske smerter

Abstract: The number of patients who received analgesics and opioids on reimbursable prescriptions was low compared to the proportion of the population with chronic pain and the proportion using opioids long-term. 38% of those reimbursed for opioids also used benzodiazepines, which is contrary to official Norwegian guidelines.

“…11;22 Gabapentin and pregabalin are indicated for neuropathic pain and fibromyalgia, 12;31;32 and low-dose cyclic antidepressants for chronic back pain, neuropathic pain, and fibromyalgia. 14;32;33 In clinical practice all are widely prescribed for chronic back and other musculoskeletal pain, 13;14;32;34 by far the most common recorded diagnosis in the study cohort. Thus, material confounding by indication seems unlikely, a conclusion supported by the essentially unchanged findings for control groups restricted to patients with a diagnosis of neurologic pain or consisting exclusively of patients prescribed either anticonvulsants or cyclic antidepressants alone.…”

“…Only 2% of the entire Norwegian population received an analgesic to treat chronic pain on a reimbursed prescription in 2010, corresponding to approximately 10% of those with a diagnosis of chronic pain at the time. 57 There is a cost limit for all payments within the public health care system, including payment for consultations and reimbursed prescriptions. Once this limit is reached, further consultations and reimbursed prescriptions are free of charge.…”

“…For alle andre aktuelle smertelindrende medikamenter (også opioider) må spesialist (ikke i allmennmedisin) søke om individuell refusjon etter § 3a til HELFO. Line Pedersen og medarbeidere oppsummerer i dette nummer av Tidsskriftet erfaringen med denne ordningen (2). Når de konkluderer med at kun 2 % av befolkningen får forskrevet smertelindrende medikamenter på blåresept, er det grunn til å stoppe opp og spørre hvorfor.…”
