Various methods and systems used to improve the quality of the management and storage of medical record files, one of which is to reduce the volume of archives that have no use value, to reduce archive buildup and create better and more efficient storage space, making it easier for officers to carry out their duties search and expedite the patient's treatment process. This study uses a qualitative type of research, with a descriptive method. The data collection in this study used the interview and observation (checklist) method. The population in this study were officers of the medical record work unit at Hermina Hospital Palembang in 2021. Based on the research, it can be concluded that the retention of inactive medical record files in 2021 is being carried out by medical record officers and the destruction of inactive medical record files has not been carried out because constrained by the COVID-19 pandemic, there are officers who carry out WFH (Work from Home) along with service hours. At the Hermina Hospital in Palembang, the standard operating procedure (SOP) regarding the destruction of inactive medical record files at the Hermina Palembang hospital already exists, but has not been implemented is 0.000 <0.05, it can be concluded that it is accepted, meaning that there is a difference in scores between before and after being treated with water tube media

Proses retensi dan pemusnahan berkas di sebuah Rumah Sakit belum dilakukan karena salah satu faktor yaitu terdapat kekurangan tenaga kerja untuk memilah berkas rekam medis yang akan diretensi. Berkas rekam medis yang memiliki nilai guna tertentu pada 7 tahun yang lalu mengalami kerusakan disebabkan oleh banyak faktor, sehingga berkas tersebut lenyap dan tidak memiliki arsip lain beserta laporannya. Akibatnya, belum terdapat perawatan yang baik terhadap berkas rekam medis tersebut, maka rumah sakit tidak dapat menyediakan berkas pada saat pasien membutuhkannya. Tujuan dari penelitian ini yaitu untuk membuat perancangan sistem informasi pengarsipan digital berkas rekam medis. Metode yang digunakan dalam penelitian ini adalah metode pengembangan sistem waterfall. Hasil dari penelitian ini berupa perancangan sistem informasi pengarsipan digital berkas rekam medis.

Hypertension or high blood pressure if a person experiences an increase in systolic blood pressure of more than 140 mmHg and diastolic pressure of more than 90 mmHg on an ongoing basis. In this study, the method used is descriptive quantitative through Community Diagnostic USG (Urgency, Seriousness, Growth), which aims to determine the existence of a health problem by collecting data from the community. The location is at RT 03 Padukuhan Druwo, Bangunharjo, Sewon Bantul, DI Yogyakarta which will be held on April 12-July 08 2022 with a team consisting of 1 lecturer and 1 student. Based on Community Diagnostics, it was found that hypertension was the main problem at the service location. Another problem is the community's non-compliance with taking hypertension drugs regularly and the community will regularly check their blood pressure, either personally to health services or during community health services. To overcome these problems, intervention was carried out in the form of direct counseling and using posters. The results obtained are that there is an increase in the average knowledge about hypertension after counseling. This is indicated by the average value of knowledge before counseling is 70 and after counseling the average value increases to 90. Appropriate interventions can provide increased knowledge and in the end are expected to increase public knowledge in preventing hypertension.
