Analysis of 82 Cases of Retroperitoneal Schwannoma

Abstract: Most of the retroperitoneal schwannomas are benign. It is difficult to make an accurate preoperative diagnosis. However, with the preoperative assessment of ultrasound-guided fine-needle aspiration, computed tomography and magnetic resonance imaging, the accuracy of diagnosis could definitely be improved. Treatment depends solely on surgery. Malignant schwannomas are insensitive to chemotherapy and radiation, resulting in poor prognosis.

“…3 Retroperitoneal schwannoma can occur in paravertebral and presacral region and is benign most commonly by nature. 4 Calcification is seen in 23% of cases and cystic degeneration in almost 60% of cases. 5 Schwannoma rarely presents with neurological symptoms.…”

“…This approach attempts to achieve a complete resection while preventing large-scale hemorrhage by isolating the vasculature before proceeding with the enucleation and removal of the tumor. 16 In cases where complete resection cannot be achieved, hemorrhages up to 2.5 L have been described in the literature. 10 Laparoscopy attempts this same anterior technique in a less invasive manner to improve postoperative quality of life.…”

“…2,3 It also becomes an important factor in planning surgical intervention that involves complete surgical resection with preservation of the integrity of the parent nerve when possible. 1,16 The shape, size, and location of these tumors as seen on CT and MRI are critical factors when choosing the surgical approach best suited for effective isolation and resection of the tumor. CT characteristics of benign retroperitoneal schwannomas include smooth tumor margins, evidence of necrosis and hemorrhage, and, often, cystic changes.…”

“…Однако, верификация шванномы, как морфологической формы затруднительна, Так, Q. Li с соавт считают, что ультразвуковое исследование, КТ и МРТ не являются специ-фичными методами диагностики шванном, так как из 82 забрюшинных шванном авторы доо-перационно верифицировали только 15,9% [14,16].…”
