Analysis of Early Complications After Radical Cystectomy: Results of a Collaborative Care Pathway

Abstract: Within the framework of a clinical pathway, radical cystectomy can be performed safely with an acceptable rate of early minor and major complications. Delay in the return of bowel function is the most common minor complication. Increased estimated blood loss, transfusion requirement and a major complication predicted a higher likelihood of postoperative ileus. The acceptable rate of early morbidity in this series in a 5-year period validates its use in patients undergoing radical cystectomy.

“…This indicates that the development of ileus may be an early predictor for the development of significant postoperative complications as reported by Chang et al [20]. Compared to other series, we report a higher rate of ileus [18,21] [22].…”

“…The University of Southern California (USC) series of 633 patients with pT2-T4a reported 5 and 10 years overall survival of 48% and 32%.¹ The Memorial Sloan Kettering Cancer Center (MSKCC) series of 184 patients with pT2-4 reported overall 5-year survival rates of 36%.² While the operative and perioperative care has improved, operative mortality in modern series ranges from 1% to 2% and postoperative complications range from 15% to 32%. 3,4 Studies confirm that the most serious side effects are urinary diversion and loss of sexual function in men and women; these effects also have a significant impact on QOL. Continent urinary procedures achieve 82% continence rates, yet complications require reoperation in 10% to 15% of these patients.…”

“…However, the rate of complications after cystectomy is significantly higher. Although the rate of minor complications is <31%, that of major complications requiring admission in a surgical intensive care unit is 4.9% [17]. Moreover, late sequelae such as erectile dysfunction and urinary incontinence are common and of concern.…”
