A modified ELISA assay was used to detect IgG binding to silicone and IgG subtypes in serum of patients bearing silicone bands, silicone sponges and intraocular silicone oil. Presence of autoantibodies (rheumatoid factor, antinuclear antibody, and SCL 70) and signs and symptoms of autoimmune diseases were also checked. Antisilicone antibodies were detected in 35.7% of patients with solid silicone and in 83% in those with silicone oil. This increase in IgG is mainly due to IgG1. Nevertheless no positive results were found in detecting signs and symptoms of autoimmune diseases.

Absence of one or two testicle due to congenital or acquired reasons creates not only physiological complications, but also cosmetic and psychological problems for males of all ages. These problems can be effectively solved by implantation of testicular prostheses. Current implants in use are made out of a silicone elastomer envelope filled either with saline, silicone gel or solid silicone elastomer. A Pubmed search was carried out for all articles on testicular prosthesis published between 1963 and 2010 for this review. The history and development of testicular prostheses, types of implants, timing of implantation, the surgical techniques and complications, and benefits of implantation of testicular prosthesis were reviewed.

IntroductionWe describe a case of spontaneous, non traumatic rupture of a single artificial testis in a patient who had undergone bilateral, staged radical orchidectomy followed by prosthesis insertion. The consequences and radiological appearances of implant rupture are discussed. We believe it is the longest time interval recorded between prosthesis insertion and rupture.Case presentationA 50 year old Caucasian man presented to our outpatient department with an altered consistency in his right testicular prosthesis without any systemic symptoms or local inflammation. His left testicular prosthesis had retained its consistency since insertion.ConclusionThe majority of cases reported to date have required exploration due to symptoms but we describe a case that was managed conservatively.

Abstract: Immunological alterations were present in 71.4% of examined recipients but they may have been coincidental. Careful followup of recipients of a silicone gel filled testicular prosthesis is needed.
