Cone beam computed tomography (CBCT) is widely used by orthodontists to obtain three‐dimensional (3‐D) images of their patients. This is of value as malocclusion results from discrepancies in three planes of space. This review tracks the use of CBCT in orthodontics, from its validation as an accurate and reliable tool, to its use in diagnosing and treatment planning, and in assessing treatment outcomes in orthodontics.

Introduction: Intraoral scanning techniques, laser scans and cone-beam computed tomography (CBCT) are becoming widely diffused in dental practice. These instruments can replace dental plaster casts with digital models; recent investigations have found that measurements taken on digital models do not differ clinically from those obtained on CBCT scans and physical models, but only intra-examiner reliability has been assessed. In the current study we tested both intra-and inter-examiner variations, together with intertechniques reliability. Methodology: Data from six adult subjects were retrospectively obtained. Twelve dental distances were measured on dental plaster casts using a digital calliper; on digital 3D CBCT images using inVivoDental software (Anatomage, San Jose, CA); and on laser scanned surfaces using Mirror Vectra Software (Canfield Scientific, Fairfield, NJ). Two different operators performed all measurements twice. Bland-Altman analysis, Kruskal-Wallis and Wilcoxon tests were used for comparisons. Results:The intra-and inter-operator biases range was 0-0.34 mm. The reproducibility range was 72-99%. The three different techniques gave very similar measurements, with biases between ±0.1 mm. Reproducibility ranged between 90 and 100%; the best reproducibility coefficients were found between plaster and digital casts, and only three differences were larger than 0.5 mm. Calliper measurements slightly overestimated digital casts values. Only the mesiodistal distance of tooth 24 (p=0.002) was significantly different among techniques. Conclusion: Measurements on digital dental models and CBCT reconstructions of the dental arches seem clinically reliable as direct calliper measurements. The inter-and intra-operator reliabilities were acceptable, some more care may be needed for CBCT measurements.

The purpose of this study was to evaluate the validity and reliability of orthodontic measurements made on digital models created with Hirox digital microscope (HDM). The data sources were plaster dental models from the archives of School of Dental Sciences, Universiti Sains Malaysia. A total of 11 dental models with 396 variables were measured using the plaster dental casts, and Hirox digital models were created. The study inclusion and exclusion criteria were followed. Two investigators (A and B) carried out the measurements of the selected dental casts using a digital caliper (DC) and the HDM. Dental plaster models were scanned using the HDM for the fabrication of the Hirox digital models. The tooth size and arch width were measured by DC and HDM. The validity of the digital measurements for examiners A and B the ICC coefficients were statistically significant (p<0.001), and the values of coefficient were in the range of strong correlation. The ICC coefficient values for inter-examiner A and B were all in the range of strong correlation for both tooth size and arch width variables in which the reliability coefficient values were comparable between digital measurements using HDM and measurements using DC. All ICC coefficient values were statistically significant (p<0.001). The results show that linear measurements of dental casts using a digital microscope are valid and reliable.
