Analysis of tumor microenvironmental features to refine prognosis by T, N risk group in patients with stage III colon cancer (NCCTG N0147) (Alliance)

Abstract: Background: Tumor-infiltrating lymphocytes (TILs), tumor budding, and micropapillary architecture may influence tumor growth and metastatic potential, thereby enhancing prognostic stratification. We analyzed these features and their relative contribution to overall outcome and in low (T1e3 N1) and high (T4 and/or N2) risk groups that are used to inform the duration of adjuvant chemotherapy in patients with resected stage III colon cancers. Patients and methods: Among 1532 patients treated in a phase III adjuva… Show more

“…46 Our previous study in colon cancer (n=1532) indicated tumours with high TILs had distinct tumour biology and were more likely to present with lower T and N stage, well or moderate histological grade, right-sided primary location, mutated BRAF V600E and deficient mismatch repair (dMMR). 47 Patients with high TILs were more likely to achieve response to ICIs, while Open access low TILs represent existing but partially exhausted antitumour immunity. 48 49 High TILs were also associated with the efficacy of chemotherapy in gastric cancer.…”

“…Therefore, our study can underscore the value of examining immune cells in tumour centre areas containing microscopic tumour invasive edges that are ubiquitous across tumour areas (regardless of gross tumour edges or tumour centre areas). A few recent studies have shown the combined measurements of tumour-infiltrating lymphocytes and tumour budding may have higher prognostic power than either measurement alone [ 56 , 57 ]. Our results are in accordance with these reports, as lymphocytic infiltrates and tumour budding had prognostic value independent of each other.…”

“…Therefore, DFS was utilized and measured from the date of diagnosis to locoregional or distant recurrence, second primary malignancy, or death, whichever occurred first. 49 Patients with no evidence of events or no follow-up information were documented as censored at the date of last contact on January 4, 2020. 48 IHC assay, WGS, and RNA-seq The steps for the IHC assay, WGS, and RNA-seq are provided in Supplementary Materials and Methods.…”
