Anaphylactic shock during cement implantation of a total hip arthroplasty in a patient with underlying mastocytosis: case report of a rare intraoperative complication

Abstract: BackgroundCemented total hip arthroplasty (THA) is a safe and common procedure. In rare cases life threatening bone cement implantation syndrome (BCIS) may occur, which is commonly caused by pulmonary embolism (PE).Case presentationWe describe the rare case of a 70-year old patient who underwent an elective total hip replacement. Before surgery he was diagnosed with underlying systemic indolent mastocytosis, a rare pathological disorder that may result in anaphylaxis after massive systemic mast cell activation… Show more

“…Our results could not point out to a significant result, because many of our patients have had intraoperative or postoperative transfusions (69.15%). As acrylic cement spacers represent a viable surgical option for many tumours [ 19 ], and it is well known that cementing a hip prosthesis for example may have an impact on the patient’s hemodynamic [ 20 , 21 ], we believe this could be something to look out for.…”

“…The third theory regarding the cause of BCIS focuses on the triggering of the immune system . The clinical presentation of BCIS mimics that of anaphylaxis, which may account for the underreporting of occurrences .…”

“…In anaphylaxis, histamine is a key trigger of cardiovascular signs and symptoms. Elevated serum histamine concentrations have been documented in hypotensive patients undergoing cemented hip arthroplasty . However, there are no studies confirming the cause of the histamine release, nor have any studies investigating the effectiveness of pretreating hip arthroplasty patients with histamine‐blocking agents provided a definitive best practice …”
