Anaplastic Thyroid Cancer: Ultrasonographic Findings and the Role of Ultrasonography-Guided Fine Needle Aspiration Biopsy

Abstract: PurposeTo investigate the ultrasonographic (US) features of anaplastic thyroid cancer (ATC) and the diagnostic performance of US-guided fine needle aspiration biopsy (FNAB) therein.Materials and MethodsEighteen cases of ATC diagnosed between January 2001 and May 2011 were included. FNAB was performed in all cases. Initial FNAB results were divided into three groups: 1) the cytological ATC group, cytological diagnosis of ATC; 2) the underestimated group, cytological diagnoses of malignancy other than ATC; and 3… Show more

“…Third, not all thyroid nodules underwent surgery or cytopathologic diagnosis. Some final references were based on cytology and follow-up US, so this can cause some false negative and false positive cytologic results 3,25. Fourth, diagnostic performance varies according to the methodology of BRAF V600E testing; therefore, the result may not be reproducible if another method is used.…”

“…Suh et al (38) reported that markedly hypoechogenicity is a common US feature in ATC (52.9%, 9/17), and Hahn et al (5) reported hypoechogenicity is more common in ATC (61.5%, 16/26). In our study, marked hypoechogenicity occurred more frequently in PTL than ATC with significantly difference, but this result still need further investigations.…”

“…Despite the rarity (1‐2 cases per million), the disease is responsible for 14%‐50% of thyroid cancer mortality. One‐ and 10‐year survival estimates are 10%‐20% and <5%, respectively . There is no effective therapy for metastatic anaplastic thyroid cancer with few randomized control trials evaluating different strategies including a combination of surgery, chemotherapy, and radiation treatment .…”

“…Our patient had a delay in diagnosis as two prior fine‐needle aspirations were nondiagnostic. It has been postulated that fine‐needle aspiration can fail due to sampling in areas burdened with necrosis, fibrosis, or hemorrhage and the overall difficulty in obtaining adequate cells from masses containing both solid and cystic components . Primary care physicians and endocrinologists should be aware that neck masses with concerning features like, in this case, a delay in evaluation and treatment may dramatically shorten a patient's life.…”
