BackgroundUnilateral weakness of an upper extremity is most frequently caused by traumatic nerve injury or compression neuropathy. In rare cases, lesion of the central nervous system may result in syndromes suggesting peripheral nerve damage by the initial examination. Pseudoperipheral hand palsy is the best known of these, most frequently caused by a small lesion in the contralateral motor cortex of the brain. The ‘hand knob’ area refers to a circumscribed region in the precentral gyrus of the posterior frontal lobe, the lesion of which leads to isolated weakness of the upper extremity mimicking peripheral nerve damage. The etiology of this rare syndrome is almost exclusively related to an embolic infarction.Case presentationWe present the case of a 70-year-old male patient with isolated left sided upper extremity weakness and clumsiness without sensory disturbance suggesting a lesion of the radial nerve. Nerve conduction studies had normal results excluding peripheral nerve damage. Neuroimaging (cranial CT and MRI) detected 3 space occupying lesions, one of them in the right precentral gyrus. An irregularly shaped tumor was found by CT in the left lung with multiple associated lymph node conglomerates. The metastasis from this mucinous tubular adenocarcinoma with solid anaplastic parts to the ‘hand knob’ area was responsible for the first clinical sign related to the pulmonary malignancy.ConclusionsPseudoperipheral palsy of the upper extremity is not necessarily the consequence of an embolic stroke. If nerve conduction studies have normal results, neuroimaging – preferably MRI – should be performed, as lesion in the hand-knob area of the precentral gyrus can also be caused by a malignancy.

Background Recently, the 2021 WHO Classification of Tumors of the Central Nervous System (fifth edition) was published. WHO CNS5 incorporates numerous molecular changes to the accurate classification of CNS neoplasms. GBM, as a special type of glioma, is confined to IDH-wildtype in WHO CNS5, which has a short medical history and poor prognosis. LncRNA HOTAIR has been found to be responsible for the poor prognosis of several human cancers in previous studies. The clinical significance, the prognostic and treatment value of HOTAIR for GBM in the new WHO CNS5 classification remain unclear.Methods We created a Random Forest-based prediction model to assess the IDH1 mutational status of patients in REMBRANDT cohorts. Then we analyzed the role and the potential mechanism of HOTAIR in TCGA, CGGA, and REMBRANDT cohorts. GSVA and GSEA analysis were used to detect the potential biological functions and signaling pathways. The infiltration of immune cells was quantified by Cibersort. The effectiveness of targeted therapy in the clinical application was sought through drug response analysis. Furthermore, we verified that HOTAIR affected the proliferative activity of that glioma cells.Results We found that HOTAIR is highly expressed in all cohorts. The patients’ samples with high HOTAIR expression have poorer overall survival. The results of the functional analysis indicated that the cell cycle and proliferation-related processes were enriched in the high HOTAIR expression group. The infiltration of immunocytes is different in the two groups. In multiple immune checkpoints, the risk score showed a strong correlation. The analysis of drug treatment response present that the high HOTAIR expression group has a better treatment response and better curative effect on the treatment of temozolomide, sorafenib, lenalidomide, bexarotene, and axitinib.ConclusionIn conclusion, We found that HOTAIR could lead to poor prognosis of GBM mainly through regulation of cell cycle and apoptosis. And HOTAIR could be used as a marker to guide chemotherapy in GBM patients rather than immunocheckpoint inhibitor therapy.

Characteristic Alterations of Network in Patients With Intraoperative Stimulation-Induced Seizures During Awake Craniotomy

Background: The use of electrocorticography (ECoG) to avoid intraoperative stimulation-induced seizure (ISS) during awake craniotomy is controversial. Although a standard direct cortical stimulating (DCS) protocol is used to identify the eloquent cortices and subcortical structures, ISS still occurs. Epilepsy is related to alterations in brain networks. In this study, we investigated specific alterations in brain networks in patients with ISS.Methods: Twenty-seven patients with glioma were enrolled and categorized into the ISS and non-ISS groups based on their history of ISS occurrence. A standard DCS protocol was used during awake craniotomy without ECoG supervision. Graph theoretical measurement was used to analyze resting-state functional magnetic resonance imaging data to quantitatively reveal alterations in the functional networks.Results: In the sensorimotor networks, the glioma significantly decreased the functional connectivity (FC) of four edges in the ISS group, which were conversely increased in the non-ISS group after multiple corrections (p < 0.001, threshold of p-value = 0.002). Regarding the topological properties, the sensorimotor network of all participants was classified as a small-world network. Glioma significantly increased global efficiency, nodal efficiency, and the sigma value, as well as decreased the shortest path length in the ISS group compared with the non-ISS group (p < 0.05).Conclusions: The specific alterations indicating patient susceptibility to ISS during DCS increased global and nodal efficiencies and decreased the shortest path length and FC induced by gliomas. If the patient has these specific alterations, ECoG is recommended to monitor after-discharge current during DCS to avoid ISS.
