Androgen Synthesis in the Gonadotropin-Suppressed Human Testes Can Be Markedly Suppressed by Ketoconazole

Abstract: Combining inhibition of steroidogenesis with gonadotropin suppression lowers IT-T more than gonadotropin suppression alone. This combination might be useful to determine the minimum IT-T concentration necessary for human spermatogenesis, information essential for developing male hormonal contraceptives.

“…9,10 The high efficacy among Chinese men has also been replicated using monthly TU injections. Limited invasive studies measuring intratesticular testosterone (and DHT) suggest that the degree of depletion may not predict reliably complete suppression of sperm output [196][197][198][199] and that other noninvasive markers of endogenous Leydig cell function, such as circulating epitestosterone 200 or 17-hydroxyprogesterone 201 or nonsteroidal testicular products such as INSL3, 202,203 may be markers of intratesticular androgen depletion. 187,188 However, the reasons for within-and between-population differences in susceptibility to hormonally induced azoospermia remain largely unexplained.…”

“…Whether these considerations are relevant in humans is not directly known. If relevant, further suppression of intratesticular testosterone by direct inhibition of steroidogenesis may be required, and proof of principle in humans has been demonstrated using a nonspecific inhibitor of steroidogenesis 49 . Drugs which inhibit 17 beta-hydroxysteroid dehydrogenase type 3 enzyme, which catalyzes the final step of testosterone biosynthesis, would specifically reduce testicular steroidogenesis.…”

“…Esetleges szerepük az azoospermia indukciójának gyorsításában merülhet fel [11], de egy TE és DMPA kombinációval végzett tanulmány során a kezelés kiegészítése a GnRHantagonista acyclinnel nem gyorsította a spermiogenezis gátlását és nem javította a spermium-célkoncentrációt elérő résztvevők arányát [36]. A GnRH-antagonisták használata egyelőre a klinikai kutatásokban való felhasználásra korlátozódik [38], bár új, per os alkalmazható hatóanyagok esetleg szerepet kaphatnak valamilyen fogamzásgátló kombinációban [11].…”
