Anemia in Children and Adolescents with Hypothyroidism

Abstract: In a review of 17 adolescents and children (excluding newborns) with definite clinical signs, symptoms, and laboratory findings of hypothyroidism, 11 patients (65%) had anemia. The mean corpuscular volume (MCV) of the red blood cells was either macrocytic or normocytic. The hemoglobin did not correlate with the serum thyroxine level. Anemia occurred only in those patients with heights below the third percentile, but there was no similar correlation with weights. Of the 10 patients who had radiographs for bone … Show more

“…As MSH acts via GPCRs, it is reasonable to speculate that streaky hyperpigmentation may be induced by differences in receptor distribution and activities in the presence of homologies and cross reaction between MSH and TSH. In the present case, the patient presented with anemia, which is, to the best of our knowledge, not common in hypothyroidism (31), while it has been noted in certain cases of VWGS (2)(3)(4)(5)7,32). It was speculated that the anemia may be associated with the reduced red cells and decreased metabolic oxygen requirement in tissues of patients with hypothyroidism (31).…”

“…Anaemia is not so uncommon in hypothyroidism (40,41) and has been noted in several case reports of the Van Wyk-Grumbach syndrome (4,6,8,9,10,18). The proposed mechanism involves decreased red cell production in response to the reduced metabolic requirements for oxygen in tissues in hypothyroidism (40,41).…”

“…The proposed mechanism involves decreased red cell production in response to the reduced metabolic requirements for oxygen in tissues in hypothyroidism (40,41). In addition, the anaemia may also be exaggerated by menorrhagia, dietary deficiency or pernicious anaemia as part of an autoimmune 'cluster' (41).…”

“…Although microcytic anaemia is not found in hypothyroid adult, it can be noted in children with hypothyroidism 9 . Furthermore, there is a relationship between the severity hormonal defect with appearance of cardiac tamponade 2 and anaemia.…”
