Anesthesia, cytokines and cancer recurrence

Abstract: Cytokines are essential mediators for the regulation of both innate and acquired immunity and hematopoiesis. They modulate immune cell signaling, activation, adhesion and functioning. They regulate the individual response to several insults such as infection, inflammation, trauma, and pain. Moreover, the balance between pro-inflammatory and anti-inflammatory cytokines is critical for the evolution of surgical complications and tumor progression.Several drugs, including anesthetic agents, influence cytokines se… Show more

“…The term "intravenous anesthetics" includes substances with anesthetic properties that are not gases and that are administered intravenously (41). Intravenous anesthesia was used in 1930, with the introduction of barbiturates, and these drugs are currently considered essential in deep sedation and general anesthesia, whether balanced with the use of inhaled agents, or total intravenous anesthesia (46).…”

“…When investigating the link between this anesthetic and the inflammatory response, significant results were obtained; thanks to their inhibitory effect on cyclooxygenase 2 (COX-2). There is less production of PGE 2 by human monocytes in vitro, (53) and it decreases the concentration of other proinflammatory cytokines, such as IL-10, IL-6, or tumor necrosis factor α (TNF-α) (46). Due to all the mentioned mechanisms, propofol generates a lower inflammatory response when compared to inhaled agents, which could improve the oncological prognosis of patients (53).…”

“…[ 4 6 ] Volatile anaesthetics have been reported to induce carcinogenic effects through impact on hypoxia-inducible factors and insulin-like growth factors, both of which are potentiated by the use of volatile anaesthetics. [ 12 13 ] Sevoflurane induces cell motility and invasion by reduction of matrix metalloproteinase (MMP)-2 and MMP-9126, and xenon inhibited migration in breast adenocarcinoma acells. [ 14 ] However, conflicting data exist about volatile anaesthetics as well.…”

“…These changes are caused by afferent neuronal stimulus from the area of surgery, cytokines release from tissue damaged by trauma, and activation of cellular and humoral immune pathways. 1,2 Both general and regional anesthesia help to suppress the stress response due to surgical trauma. This happens because anesthesia provides sympathetic and afferent nerve fibers block that modulate the pituitary-adrenocortical system.…”
