Angiofibrolipoma of the Ear Canal

Abstract: This variant of lipoma contains mature adipocytes, blood vessels, and dense collagenous tissue. The physical examination can be misleading, and the diagnosis requires histopathological examination. The patient was treated with complete surgical excision, tympanoplasty, canalplasty, and skin grafting to the external auditory canal. His pure-tone average improved from 37 to 11 dB, and his air-bone gap was closed completely. The lipoma has not recurred in the 6-month period following surgery.

“…With respect to the ear, lipomas arising from the external auditory canal can be misleading and often imaging in the form of a CT scan of the temporal bones is needed to confirm diagnosis, especially if rare lipoma variants, such as angiofibrolipomas 5. In our case, the pinna arose from the subcutaneous tissues and diagnosis was straightforward despite its unusual location.…”

“…The other histological variants of lipomas include fibrolipomas, angiolipomas, angiomyolipomas, and infiltrating angiolipomas. 4 This classification is based on the relationship between the fat, muscle, blood vessel, and connective tissues involved in the tumor structure.…”

The growths are not encapsulated but are histologically distinct from surrounding tissues.…”
