A 13-year-old boy with a solitary exostosis of the left femur was seen with a pseudo-aneurysm of the popliteal artery. When left leg pain occurred 3 months earlier, radiographic examination revealed an exostosis with a cartilage cap. Serial radiographic examination demonstrated gradual disruption of the cartilage cap of the exostosis as the pseudoaneurysm developed. An exostosis with an irregular surface was found at surgery. A literature review disclosed 39 similar cases in which loss of the cartilage cap was considered as one of the causes of the aneurysm formation. Considering the clinical course of our patient, however, we believe that exostoses lose their cartilage caps by pressure destruction due to the aneurysms. It is highly probable that loss of the cartilage does not cause the aneurysms.

“… 3,10,14,16,20–25 The majority of these pseudoaneurysms tend to occur in the popliteal fossa as a consequence of the inherent frequency of osteochondromas to arise at the knee, coupled with local vascular anatomical factors at this site, namely the relative immobility of the popliteal artery as it emerges from the adductor canal. 4,5,9,19,22,24–28 The presenting patient did not have a palpable popliteal pulsatile mass pre‐operatively and, indeed, no pseudoaneurysm was observed at surgery, although some degree of poststenotic dilatation of the vessel was present. Popliteal venous compression and thrombosis are also recognized complications of osteochondroma but these are less frequently encountered than are the arterial sequelae.…”

Osteochondroma is the most common, benign, skeletal tumour occurring in 1% of the population and being more prevalent in males.1,2 Mostly metaphyseal in location, they develop from cortical bone hyperplasia at the epiphyseal plate.3 They are often incidental findings on plain radiographs during adolescence although, rarely, may be associated with vascular complications most often affecting the popliteal artery. 4,5 We describe a case of pseudo-aneurysm of the popliteal artery secondary to an osteochondroma of the femur in a young adult. Case reportAn 18-year-old man was referred to our institution with a 2-year history of swelling in the right popliteal fossa following minimal blunt trauma during a football match. A rapid increase in size had been noted over the preceding 2 months.A 10-cm, non-pulsatile, painless mass was found in the right popliteal fossa. Venous obstruction was evident with distal oedema and varicosities. There was no evidence of neurological compromise and distal pulses were palpable.Plain radiography revealed an osteochondroma arising from the distal femoral shaft without evidence of fracture (Fig. 1) A case of pseudo-aneurysm of the popliteal artery secondary to an osteochondroma of the femur in a young adult is described.
