Angiolipoma in the Head and Neck: Imaging, Diagnosis and Management

Abstract: Angiolipoma, distinguishable from other lipomas by its excessive degree of vascular vessels, are rare in the head and neck and require unique management. A slow growing mass, located underneath the inferior border of the right mandibular angle of a 51-year-old female, was excised under general anesthesia. Unexpected excessive bleeding during the excision was observed and the histological specimen was diagnosed as angiolipoma. As shown in this case report, pre-operative imaging modalities have a crucial influen… Show more

“…Angiolipoma is a variant of lipoma that includes mature lipocyts and blood vessels (2)(3)(4)(5)(6). In different cases, the ratio of the lipocyt to the blood vessels varies, but in most of them the percentage of lipocyts is higher than blood vessels (4,8).…”

“…In the maxillofacial region, due to the existence of many vital structures, magnified operation is necessary to prevent damage to these structures, but in some situations sacrificing a branch of nerves is inevitable (12). According to previous studies, the relapse rate of infiltrating angiolipoma is 35-50% (6). Two possible causes of the recurrence of infiltrating angiolipoma after surgery are incorrect evaluation of the extent of the tumor and inadequate tumor resection during surgery (5).…”

“…Angiolipomas are benign mesenchymal tumors composed of mature adipocytes and are classi ed into nonin ltrating/encapsulated or in ltrating/unencapsulated which are less common. 1 In ltrating angiolipomas, though rare, may involve the invasion or encroachment of adjacent structures and can lead to additional symptoms, such as nerve compression and are more likely to recur due to di culty in complete surgical removal. 1 Angiolipomas had been identi ed in patients with HIV (PWH) on antiretroviral therapy (ART) well before integrase strand transfer inhibitors (INSTIs), such as bictegravir, were available.…”

“…Infiltrating angiolipomas lack a circumferential capsule and are characterised by adjacent structure invasion and difficulty separating the masses from the surrounding tissues ( 8 ). Although benign, inadequate excision may lead to recurrence ( 9 ). Infiltrating angiolipomas appear more frequent in elderly patients ( 9 ).…”
