Anomalous origin of the entire coronary system by three separate ostia within the right coronary sinus – a rarely observed coronary anomaly

Abstract: A 56-year-old man, who had been followed up in the heart function clinic at St. Paul's Hospital (Vancouver, British Columbia) for ischemic cardiomyopathy, presented complaining of left-sided chest heaviness that improved with nitroglycerin. In 1998, he had an inferior myocardial infarction that was treated medically. He had a coronary angiogram at that time, the report of which was not available; however, the test did show anomalous coronary arteries. A myocardial perfusion scan showed a fixed inferior wall de… Show more

“…Anomalous origin of all the three coronary arteries originating from the right coronary sinus is a very rare anomaly; only a few cases are reported in the medical literature. [ 6 9 ] The course of the arteries that originate from the right coronary sinus is variable. The RCA follows the usual pathway to the right atrioventricular sulcus.…”

“…In the literature, coronary arteries originating from the right sinus of Valsalva with a single ostium have been reported, but there are only a few cases wherein all three coronary arteries originate separetely from the right coronary sinus. [ 6 7 8 9 ] We present the case with MDCT and catheter angiographic findings.…”

“…In the literature, CAA cases where 3 coronary arteries originated from separate ostia in RSV were seen in 7 patients in the study of Kashyap et al, in 3 patients in the study of Clik et al, in 1 patient in the study of Engel et al, and in 1 patient in the study of Yamanaka and Hobbs [1,4,9,10]. Al-Mohaissen et al reviewed 34 cases from the literature and revealed that RCA often had a normal course, Cx mostly ran behind the aortic root and rarely in front of the pulmonary artery or at the interventricular septum; the LAD mostly ran in front of the pulmonary artery, septum or the interatrial septum [11].…”
