Anorexia nervosa in cystic fibrosis

Abstract: This article explores the challenges associated with diagnosing and managing eating disorders such as anorexia nervosa amongst adolescents and adults with cystic fibrosis. It reviews the known risk factors, generic verses disease specific eating disorder risk screening tools and considers the ethical dilemmas associated with critically low body mass indices. A case review is included to illustrate the complexities of managing both conditions in the context of declining respiratory function.

“…In addition, those patients with CF also add malabsorption due to the associated impairment in exocrine pancreatic function (42). A relevant symptom that frequently appears in chronic respiratory diseases is anorexia (43)(44)(45), defined as a reduction or loss of appetite. This symptom can be related to the lung disease itself but also to the treatment and/or the accompanying emotional distress, and can worsen nutritional abnormalities.…”

“…Second, it is also the case that sampling female adolescents and young adults with eating disorders (Collin et al., 2016; Cornelius & Blanton, 2016; Macleod, 1981; Pelletier Brochu et al., 2018) carries a risk of including those with additional psychiatric and somatic issues. Indeed, there is a considerable overlap between eating disorders and perfectionism (Johnston et al., 2018), complex multiple physical symptoms and comorbid psychiatric disorders (Cooney et al., 2018; Marucci et al., 2018), body image disturbances (Lewer et al., 2017), obesity (He et al., 2017), cystic fibrosis (Linkson et al., 2018), and Type I Diabetes (Toni et al., 2017). It is therefore quite possible that latent confounders biased the results of the studies referred to above.…”

“…Table 1 describes all available primary research evaluating disordered eating attitudes and practices and the prevalence of EDs. Most studies have adopted a cross-sectional design [ 16 , 17 , 18 , 19 , 20 , 21 ], with few case-control studies [ 22 , 23 , 24 , 25 , 26 , 27 , 28 ] and case reports [ 8 , 29 , 30 , 31 ]. Research originates mainly from the UK [ 8 , 16 , 17 , 18 , 20 , 21 , 22 , 23 , 30 ], the USA [ 19 , 25 , 26 , 27 , 31 ], Canada [ 24 , 29 ], and Australia [ 28 ].…”

“…Nevertheless, several additional comorbidities appear to change these dynamics, and visibly apparent factors, including scars, ports, and tubes, and the need for supplementary oxygen supply, may also influence body image perception [ 11 ]. For instance, in CF-related diabetes, women have been dissatisfied with their body appearance, exhibiting low self-esteem [ 30 ]. When patients with CF receiving no intervention were compared to those on a nasogastric tube or receiving oral nutrient supplementation, the first exhibited more aggravated dieting behavior [ 22 , 23 ].…”
