Anterior instrumentation for cervical spine tuberculosis: An analysis of surgical experience with 61 cases

Abstract: Anterior reconstruction using titanium plates and locking screws for stabilization of the subaxial and cervicodorsal region tuberculosis is a useful adjunct in preventing kyphotic deformity. A satisfactory segmental stability and fusion is achieved by this technique.

“…During the traction process, observing the patient's tolerance and nerve function is needed so as to decrease acute spinal cord stretch during the surgical reduction process, and thus greatly avoid possible damage of spinal cord during the reduction process. Adequate bone graft is crucial for decreasing postoperative recurrence and internal fixation failure of cervicothoracic junction tuberculosis [22]. This group of patients underwent anterior and posterior combined operation: posterior bone fusion needed a large amount of bone, thus allogeneic bone could be chosen; for anterior, it would be better to choose ilium with cortex on three surfaces to implant into the titanium mesh support.…”

“…Bormann and Dunn reported neck pain and stiffness in all their serial case patients [3]. More than 50% patients reported muscle weakness [14]. They report 1 patient showing muscle weakness in left arm and alteration of sensation and reflex.…”

“… 10) Anterior surgery remains the mainstay treatment of cervical tuberculosis with instability, allowing for the debridement and evacuation of an abscess with stabilization of the anterior collapsed segment. 8 10) Posterior surgery is rarely used in lower cervical tuberculosis unless there is a posterior abscess. 9) Our case showed destruction of the vertebral body with posterolisthesis and a prevertebral abscess, so we performed an anterior decompression and stabilization.…”
