Anterior opening wedge osteotomy of the proximal tibia for anterior knee pain in idiopathic hyperextension knees

Abstract: We analysed 20 patients with 24 knees affected by idiopathic genu recurvatum who were treated with an anterior opening wedge osteotomy of the proximal tibia because of anterior knee pain. We managed to attain full satisfaction in 83% of the patients with a mean follow-up of 7.4 years. The mean Hospital for Special Surgery score was 90.3 (range 70.5-99.5), and the mean Knee Society score score was 94.6 (70-100) for function and 87.7 (47-100) for pain. The mean Western Ontario and McMaster University Osteoarthri… Show more

“…Anteriorly based osteotomies tend to increase the tibial slope, whereas a more posteriorly based osteotomy can potentially negate the changes seen in the tibial slope. 16 Noyes has shown through his computerized model that the gap angle formed needs to be twice as large in the posterior tibia than the anterior tibia to maintain the native tibial slope. 12 Many studies 4,9 have documented the changes that occur in knee stability with changes in the tibial slope.…”

“…Generally, patella infera has been described after total [13][14][15][16] and unicompartmental knee replacement [17], high tibial osteotomy [11,[18][19][20], and anterior cruciate ligament reconstruction [21]. Whereas, patella alta has been described in neurological disorders [22], in association with anterior knee pain [23], and femoropatellar instability [24].…”

“…In 2006, van Raaij et al 62 proposed an anterior opening wedge osteotomy of the proximal tibia to treat anterior knee pain in knees with idiopathic hyperextension. The authors supposed that the osteotomy could end the repetitive impingement by readdressing knee extension.…”
