Anthracycline-Induced Cardiotoxicity in Children with Cancer

Abstract: The fact that anthracyclines are cardiotoxic seriously narrows their therapeutic index in cancer therapy. The cardiotoxic risk increases with the cumulative dose and may lead to congestive heart failure (CHF) and dilated cardiomyopathy in adults and in children. The prevention of anthracycline-induced cardiotoxicity is particularly important in children who can be expected to survive for decades after being cured of their malignancy. Attempts to reduce anthracycline cardiotoxicity have been directed towards: (… Show more

“…This commonly presents as an insidious, asymptomatic decrease in LV function, with resultant cardiomyopathy, conduction defects, myocardial infarction, hypertension and exercise intolerance later in life. The adverse cardiac effects of anthracyclines are thought to be progressive and irreversible [9][10][11].…”

“…Furthermore, Stage III therapy entails additional chemotherapy (doxorubicin) and radiation therapy. Both of these therapies are associated with late effects including secondary malignancy, pregnancy related complications, and impaired cardiac and thoracic function [1214]. However, if these patients were given prenephrectomy therapy to reduce the size of the tumor and subsequent risk of IOS, they still would receive doxorubicin and flank radiation, but the risk of IOS and therefore risk of local recurrence may be lower.…”

“…Doxorubicin is one of the most effective chemotherapeutic agents against malignancies such as breast and prostate cancers and childhood leukemia (Iarussi, Indolfi 2005), but its use is associated with dose-dependent development of cardiomyopathy (Elbl, Hrstkova 2003). Myocardial injury is caused by induction of myocyte apoptosis , Kotamraju, Konorev 2000, Wang, Konorev 2004, but other mechanisms play a role such as lipid peroxidation, inhibition of nucleic acid and protein synthesis, release of vasoactive amines, changes in adrenergic function, abnormalities in calcium handling, reduced expression of specific genes, impairment of mitochondrial creatinine kinase and induction of nitric oxide synthase (Minotti, Menna 2004).…”
