Anti‐inflammatory effect of an insulin infusion in patients on maintenance haemodialysis: A randomized controlled pilot study

Abstract: During haemodialysis, insulin may have a modest anti-inflammatory effect as evident by a reduction in CRP that appears to have a persistent effect over the next 24 h post dialysis. More studies are required to examine longer-term benefits as well as the potential role in more high-risk individuals.

“…Previous reports on changes in CRP levels during HD are inconsistent as some studies show an increase [32,51,52] and others no change [6,16,53]. The concentrations of IL-6 and TNF-α were unchanged in our study, whereas IL-1β concentrations fell.…”

“…In comparison, a recent cross-over study showed that compared with a 4-h HD session conducted during fasting but preceded by a breakfast meal, the addition of low-dose insulin infusion (Actrapid 2 IU/h) during HD resulted in a reduction in plasma CRP, whereas IL-6 and TNF-α levels were unaffected [32]. …”

“…IR in patients on maintenance HD was ameliorated after 12 weeks of treatment with angiotensin II receptor blockers [131]. However, insulin per se may have a modest anti-inflammatory effect as evidenced by a reduction in serum CRP that appears to have a persistent effect over the 24 h after dialysis [132]. …”

“…Other adverse effects of hyperglycemia are its associated cardiovascular risks [24][25][26] and pro-thrombotic [27,28] and pro-inflammatory effects [29,30]. It may be noted at this point that other studies have shown anti-inflammatory effects of insulin infu-sions in hospital settings [31,32] and during the course of HD treatments [33]. However, this requires additional research and had not been a subject of this investigation.…”
