This study was carried out to evaluate the anti-inflammatory effect of losartan in 18 female rats which were divided into 3 groups. Respiratory disease were experimentally induced by the intraperitoneal injection and spray inhalation of ovalbumin (OVA) in first and second group while third group left as negative control group. First group were treated with losartan orally at dose rate of 5 mg/kg body weight, on the other hand second group considered as positive control group. ELISA test were used to estimate the concentration of TNF-α, IL-4 in BALF and total IgE in serum samples. Total WBC, neutrophil, eosinophil and lymphocyte were counted in bronchoaleveolar lavage (BAL). First and second rat groups show signs of pulmonary disease. Losartan treated group showed significant (p<0.05) decrease in concentration of TNF-α (152.483 pg/ml), IL-4 (39.733 pg/ml) in BALF and total serum IgE (56.006 pg/ml) in comparison with positive control group. A significant decrease (p<0.05) were also detected for total WBC (101.33 x10 3), neutrophil (8.83 x10 3), eosinophil (15.50 x10 3) and lymphocytes (15 x10 3) in BALF of losartan treated rat group in compare to positive control group.

BackgroundThe hallmark of COPD is chronic airway inflammation, which may be mediated by renin–angiotensin system. The renin–angiotensin system blockers such as angiotensin-converting enzyme inhibitors (ACEi) and angiotensin II receptor blockers (ARBs) have exhibited anti-inflammatory and immunomodulatory effects in patients with various diseases. We explored the effects of ACEi and ARBs on the risk of pneumonia in patients with COPD.MethodsA nested case–control study was performed on COPD patients recruited from January 2010 to August 2013 in two referral hospitals in Korea. A total of 130 COPD patients admitted with pneumonia were included, and 245 COPD patients without pneumonia were selected as controls from a total of 1,646 such patients. Controls were matched with test patients by age, sex, and severity of airflow limitation. The effects of ACEi/ARBs use on the odds ratio (OR) for the development of pneumonia were tested through conditional logistic regression.ResultsElderly patients (over 70 years of age) constituted ~30% of each group; most of the patients were male (85%). Of the COPD patients with pneumonia, 21.5% had taken ACEi/ARBs for a mean of 9.8 months (standard deviation ±3.5 months). The proportions of ACEi/ARBs users and the mean duration of such use did not differ when compared to those of the control patients (26.9%, P=0.25; 9.6±3.6 months, P=0.83). Univariate analyses indicated that the use of ACEi/ARBs was not associated with a decreased risk of pneumonia (OR =0.70, 95% confidence interval 0.41–1.23, P=0.21), whereas both a history of pulmonary tuberculosis (OR =1.85, 95% confidence interval 1.12–3.06, P=0.02) and exposure to systemic steroids (OR =2.33, 95% confidence interval 1.28–4.23, P=0.005) did show an association. After adjustment for a history of tuberculosis, comorbid chronic renal disease, and exposure to corticosteroids, ACEi/ARBs reduced the risk of pneumonia in COPD patients (OR =0.51, 95% confidence interval 0.27–0.98, P=0.04).ConclusionThis study revealed that the use of ACEi/ARBs was associated with reducing the risk of pneumonia in patients with COPD. Further prospective studies are necessary to confirm the protective effect of ACEi/ARBs and elucidate the underlying mechanisms in COPD patients.

Accumulation of advanced glycation endproducts (AGEs) has been found in healthy aging persons and during elevated glucose concentrations. It has been implicated in microvascular and macrovascular damage and delayed wound healing associated with diabetes, presumably through vascular, neurological, or intermediary metabolic modifications. The aim of the present work is to examine the potentially protective effect of telmisartan against diabetes-induced peripheral neuropathy in rats. Diabetic rats (Streptozotocin, STZ 50 mg/kg) were treated with vehicle or telmisartan (5 mg/kg) for 8 weeks. Behavioural tests (footprint and hotplate) were performed every 2 weeks after starting treatment. After 4 weeks of treatment, a wound was induced in the right hindlimb of rats and the wound size was measured every 3 days. At the end of the study, animals were sacrificed, blood, serum and hindlimb tissue were separated and used for further analyses. STZ treated rats showed a decrease in body weight, insulin, adiponectine level and soluble form of receptor of advanced glycation (sRAGE) expression and an increase in glucose, glycated haemoglobin A1c (HbA1c), tumor necrosis factor-α (TNF-α), advanced glycation end products level (AGEs) and receptor of advanced glycation end products (RAGE) expression compared to control group. This was associated with impaired performance in behavior tests and delayed wound healing. Treatment with telmisartan resulted in significant increase in body weight, serum adiponectin, serum sRAGE level and decreased TNF-α, insulin level, serum AGEs and RAGE compared to diabetes. Telmisartan lead to improvement only in gait base and intrastep distance in footprint parameters, while didn't improve hot plate latency or delayed wound healing. In conclusion, telmisartan may partially improve the deleterious effects of diabetes as peripheral neuropathy through its anti-glycation and anti-inflammatory effects
