Anti-NMDA receptor encephalitis in Japan

Abstract: 1) These findings indicate that "juvenile acute nonherpetic encephalitis" or a subset of this disorder is mediated by an antibody-associated immune response against NR1/NR2 heteromers of the NMDA receptor (NMDAR). 2) Our patients' clinical features emphasize that anti-NMDAR encephalitis is severe but potentially reversible and may precede by years the detection of an ovarian teratoma. 3) Although recovery may occur without tumor removal, the severity and extended duration of symptoms support tumor removal.

“…Los síntomas son de instauración aguda o subaguda (9), con fases identificables (10,11). En el 70 % de los pacientes se inicia con una fase prodrómica de síntomas inespecíficos, como náuseas, diarrea, fiebre y cefalea.…”

“…This is clear in paraneoplastic cases where a cross-reactive immune response to neural antigens expressed by a tumour leads to neuronal dysfunction. In idiopathic cases it is possible that an occult tumour may have been eliminated by immune surveillance mechanisms that subsequently generate a neurological syndrome or even that a tumour may not be detectable until much later: anti-NMDAR encephalitis may precede by years the detection of an ovarian teratoma ( (Iizuka et al 2008). A preceding infection may also provoke an antibody-mediated autoimmune response as demonstrated in patients with Guillain Barré syndrome (Willison et al 2002).…”

“…Varying rates of tumour presence at diagnosis of NMDAR encephalitis have been recorded. Dalmau et al reports this to be about 25% 9 while others report much lower rates 21,22 . Florence et al studying 81 patients found that 30% of women younger than 18 years, and less than 10% of girls younger than 14 years had tumours 12 .…”
