Anti‐TNF therapy is not associated with an increased risk of post‐colectomy complications, a population‐based study

Abstract: This large population-based study does not demonstrate any association between preoperative anti-TNF therapy and postoperative complications in UC patients undergoing subtotal colectomy. The only variables associated with complications were colectomy performed during an unplanned admission and smoking.

“…The risk of postoperative infectious complications in patients receiving biologics with immunosuppressive function before surgery may be high. Recently, multiple studies (814) have evaluated the impact of preoperative biologic therapy on the incidence of complications after surgery for IBD, but the findings of these studies are not consistent. A systematic review has found that preoperative anti–TNF-α therapy modestly increases the risk of complications after intestinal surgery for either UC or CD (15).…”

“…Test for subgroup differences: Chi 2 = 3.00, df = 2 (P = 0.22); I 2 = 33.3% Wound infection. Among studies with a preoperative anti-TNF-a window within four weeks or more than four weeks, 27,32,39 the data revealed no significant difference in wound infection between the two cohorts (p ¼ 0.93; OR ¼ 0.98; 95% CI 0.57-1.68, I 2 ¼ 0%; Figure 6(b)).…”

“…In this issue Ward et al . have made a large register based study of 6 225 patients with UC going through colectomy, comparing those with biologicals pre‐operatively (12%) with those not. No data regarding mortality is presented but biologicals were not found to be associated with an increased risk of post‐operative complications.…”
