The objective of this paper was to determine the effect of tamsulosin use on plasma insulin status in benign prostatic hyperplasia (BPH) patients in Sokoto, Nigeria. Standard methods and procedures were used. At 2nd and 3rd months of tamsulosin use, there was a significant increase (P<0.05) in plasma insulin values compared to baseline. Likewise, at 4th month of tamsulosin use, a significant increase (P<0.05) in plasma insulin values was revealed compared to baseline and 2nd month values. Other inter-mean values comparisons were not significantly different (P>0.05). Plasma insulin at 4th months of the study in BPH patients (45-54 years) treated with tamsulosin revealed a significant increase (P<0.05) compared to baseline values. Other inter-mean values comparisons were not significantly different (P>0.05). BPH patients (aged 55-64 years) at 3rd month showed a significant increase (P<0.05) in plasma insulin values compared to baseline values. Likewise, at 4th month of tamsulosin use, a significant increase (P<0.05) in plasma insulin values was seen compared to baseline and 1st month values. Other inter-mean values comparisons were not significantly different (P>0.05). In BPH patients aged 65 and above, there was no significant difference (P>0.05) in plasma insulin values from 1st through 4th month compared to baseline and other inter mean values comparisons. Plasma insulin at 0th , 1st , 2nd , 3rd and 4th months of BPH patients of different age groups treated with tamsulosin revealed no significant differences (P>0.05). This study revealed that tamsulosin use in the patients lead to significant increase in plasma insulin level (hyperinsulinemia). Careful and routine monitoring of BPH patients should be done. Patients need enough exercise activities and some drugs can be administered to alleviate elevated glucose or insulin. Insulin autoimmune syndrome (IAS) is a rare cause of hyperinsulinemic hypoglycemia characterized by antibodies to endogenous insulin without exposure to exogenous insulin. In this report, we presented a case of insulin autoimmune syndrome with a history of fasting hypoglycemia. After work up and exclusion of other causes such as insulinoma, hyperinsulinemic hypoglycemic state of the patient was considered to have been induced by etofenamate. Although IAS is generally self-limiting and dietary management and withdrawal of trigger drug are enough to maintain euglycemia, in some cases corticosteroids, plasmapheresis, rituximab can be used for treatment. In our case, despite dietary management, hypoglycemia was severe and the patient's life quality was adversely affected. After treatment with prednisolone, hypoglycemic episodes became less and less frequent. IAS should be considered as a differential diagnosis of hyperinsulinemic hypoglycemic states to avoid unnecessary interventions.

Effects of Co‐administration of Sulfonylureas and Antimicrobial Drugs on Hypoglycemia in Patients with Type 2 Diabetes Using a Case‐Crossover Design

Objectives The purpose of this study was to investigate the effect of sulfonylureas (SUs) and antimicrobial co‐administration on hypoglycemia in patients with type 2 diabetes mellitus (T2DM). Methods We conducted a case‐crossover study using the Korean Health Insurance Review and Assessment Service‐National Inpatient Sample database, using data from 2014 to 2016. Hospitalized adult patients with T2DM who were diagnosed with hypoglycemia and prescribed SUs for at least 120 days were included. Different risk ratings of severity of drug–drug interactions were considered, including “level X, D, or C” in Lexi‐Interact online and “contraindicated, major, or moderate” in Micromedex. Exposure to antimicrobials in the 30‐day period before the first hypoglycemia diagnosis was assessed. Two control periods (61–90 and 91–120 days) were matched before the diagnosis date. Conditional logistic regression analysis was conducted to compare the odds of antimicrobial exposure. Results A total of 9339 patients were included. The mean age of the patients was 71.3 ± 10.6 years, and 4818 (51.6%) were women. An increased risk of hypoglycemia was associated with co‐administration of SUs and certain antimicrobials (adjusted odds ratio [aOR] 2.56, 95% confidence interval [CI] 2.34–2.80). The antimicrobial agents that were associated with an increased risk of hypoglycemia, when co‐administered with SUs, were sulfonamides (aOR 2.99, 95% CI 1.99–4.52), fluoroquinolones (aOR 2.62, 95% CI 2.38–2.89), macrolides (aOR 2.48, 95% CI 1.88–3.27), and tetracyclines (aOR 1.56, 95% CI 1.05–2.33). Conclusions Co‐administration with SUs and certain antimicrobials increased the risk of hypoglycemia. Thus, clinically relevant interactions in patients concurrently using SUs and antimicrobials should be monitored, especially within 30 days after co‐administration.
