Anti-tumour necrosis factor therapy is associated with certain subtypes of chronic rhinosinusitis

Abstract: This is the first observational study indicating increased prevalence of chronic rhinosinusitis in patients treated with anti-tumour necrosis factor therapy. These clinical findings require investigation in greater depth to clarify the nature of pathologies currently diagnosed and treated as chronic rhinosinusitis.

“…16,19,20 Historically, SIDs have been less studied in the context of CRS but are known to be associated with sinonasal disease. 33,51,52 With the number of patients on immunosuppressive medications increasing, it is equally important to understand the sinonasal treatment outcomes in this patient population. 16 Unfortunately, standardized treatment guidelines for CRS in adults with…”

“…TNF-a inhibitors have been shown to be associated with new onset CRS, with rates in the literature ranging from 1% to 20%. 5,9,10 Wang et al performed a retrospective chart review examining CRS in patients on TNF-a inhibitors. Of the patients who were diagnosed with CRS on TNF-a inhibitors, 57% of those patients were diagnosed with new onset CRS since the initiation of TNF-a inhibitors.…”

“…The agents best studied in the literature are tumor necrosis factor alpha (TNF-a) inhibitors, which have been shown to be associated with new onset CRS, with rates in the literature ranging from 1% to 20%. 5 Even less is known about other immunosuppressive agents such as diseasemodifying antirheumatic drugs (DMARDs). Through this work, we seek to add to the growing body of knowledge of CRS endotypes by performing a structured histopathologic analysis of patients with CRSi.…”

“…The number of patients in the study is small and there was no control; nevertheless, the authors report no side effects and consider the method safe. Further work on this is required, as the need to manage this difficult group of patients 7 is clear.…”
