Introduction: This review aimed to provide a critical appraisal of alternative antimicrobial strategies in lieu of traditional triple antibiotic paste (TAP). Methods: This review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. The literature search was performed in 8 databases (PubMed/Medline, Embase, LILACS, Web of Science, Scopus, BVS, SciELO, and the Cochrane Library), selecting clinical, in vitro, in vivo, and in situ studies that evaluated antimicrobial alternatives to TAP in regenerative endodontics. Studies lacking an experimental TAP group were excluded. Results: A total of 1705 potentially relevant records were initially identified. From the 38 studies retrieved for fulltext reading, 16 fulfilled all selection criteria and were included in the qualitative analysis. According to the study design, 11 studies were solely in vitro, 1 study was both in vitro and in vivo (animal model), 2 studies were solely animal experiments, and 2 studies were clinical trials. The alternative antimicrobial agents to TAP consisted of modified TAP formulations (eg, a combination of TAP with chitosan); TAP-eluting nanofibers; propolis; chlorhexidine (CHX) gels/solutions; double antibiotic pastes composed of distinct combinations of antibiotics; Ca(OH) 2-based formulations; and sodium hypochlorite. Overall, most of the alternative agents performed similarly to TAP, although some strategies (eg, Ca(OH) 2-and CHX-based formulations) seemed to present dubious importance in the control of infection. Conclusions: TAP still remains an excellent option in terms of the complete elimination of microorganisms. This review points to the use of electrospun fibers as a drug delivery system to offer a controlled release of the antimicrobial agent, as well as the use of natural compounds, deserving future investigation. KEY WORDS Antimicrobial agents; natural compounds; regenerative endodontics; scoping review Regeneration of pulp tissue holds the promise of prolonged use of the natural dentition through predictable maintenance of dental root structure. Although the field of endodontics has been aware that healthy pulp tissue exists within infected/inflamed pulp, resective therapy has been the standard of care 1. However, in cases where pulp necrosis occurs in permanent teeth with open apex, the most commonly used treatment is apexification 2. This technique is based on the use of calcium hydroxide [Ca(OH) 2 ] or mineral trioxide aggregate (MTA) to induce hard tissue apical barrier formation 3 and presents success rates that vary from 26% to 100%. Background: There has been an increased use of natural products as medicament in endodontic treatment due to the increased adverse effect observed in chemical products used. The natural products used aims to disinfect the root canal and provide a sterile environment as efficiently as the comparable chemical products. Aim: To compare the antibacterial efficacy of cinnamon extract and calcium hydroxide as intracanal medicament against E. fecalis. Materials and Methods: The study is an in vitro randomized controlled study conducted on 40 single rooted permanent teeth. The study sample was divided into two groups-cinnamon extract as intracanal medicament and calcium hydroxide. The intracanal medicament was placed for 7 days. The outcome was measures using colony forming units per ml. Statistical analysis was done using Mann-Whitney's test to compare between the two groups. Results: Cinnamon extract intracanal medicament showed higher antibacterial efficacy as compared to calcium hydroxide. (p=0.002). Conclusion: Cinnamon extract showed better antimicrobial efficacy against E. feacalis as intracanal medicament as compared to calcium hydroxide.

OBJECTIVE: To compare the efficacy of calcium hydroxide, triple antibiotic paste and double antibiotic paste in reducing bacterial load and post treatment pain in symptomatic apical periodontitis. METHODOLOGY: In this comparative cross sectional study, 30 patients were divided into three groups. After sample collection Group A (control group) was medicated with calcium hydroxide (CH) paste, Group B with triple antibiotic paste (TAP) and Group C with double antibiotic paste (DAP). On day seven, pain status was checked by using visual analogue scale and canals were washed and another bacterial sample was taken and sent to laboratory to evaluate bacterial count. Evaluation of bacterial count and pain was done. Data analysis was performed in statistical packages for social sciences (SPSS) version 20.0. Mean & SD was calculated. Paired T-test was applied to compare qualitative data of control group with other two groups and P value (< 0.05) perceived as significant. RESULTS: All baseline samples were positive for bacteria with considerable pain. Following 07 days post-treatment VAS (visual analogue scale) showed mean value of 2.5±1.269 for CH, 1.7±0.823 for TAP and 2.0±1.054 for DAP. In post-treatment presence of Bacteria, mean was 1.1±0.316 for CH, 1.3±0.483 for TAP and 1.2±0.422 hence post treatment bacteria presence was lower in TAP and DAP and was higher in CH group. CONCLUSION: From the results it is concluded that DAP provided better antimicrobial effect with significant decrease in pain and bacterial count in comparison to control group and almost equal in efficacy to TAP.
