Antibiotic Resistance in Streptococcus pneumoniae: What Does the Future Hold?

Abstract: The recent emergence of strains of drug-resistant Streptococcus pneumoniae (DRSP) is a serious clinical and public health problem. Several interventions have been proposed to limit the further emergence and spread of DRSP, including campaigns for appropriate antibiotic use and the introduction of pneumococcal conjugate vaccines. Whether the current epidemic of drug resistance in S. pneumoniae is sustainable or will succumb to current efforts to limit its spread will be decided by an interaction of factors rela… Show more

“…Serotype 3 is an important cause of adult pneumococcal disease that is not represented in the PPS-based conjugate vaccine presently used for children (32,33). Hence, antibody-based approaches to treatment of serotype 3 infections may be needed as an adjunct to present therapies in light of the continuing problem of pneumococcal drug resistance and the phenomenon of serotype replacement in PPS-vaccinated children (39,50). The treatment of resistant pneumococcal strains is an area of critical need.…”

“…Antimicrobial resistance can be associated with a decrease in bacterial fitness (3,27). A measure of fitness of antimicrobial-resistant pathogens could aid in the prediction of the future rates of disease caused by these bacteria, guide recommendations for empiric therapy for some bacterial infections, and direct the development of new antimicrobial drugs.…”

“…Because nasopharyngeal colonization is the key event leading to cross-transmission on one hand and infection on the other (15,20), it appears to be extremely important to analyze the factors responsible for S. pneumoniae transmission in the population, like the impact of resistance described below. These factors also include high antigenic diversity, with about 90 serotypes described to date, and the effects of public health measures, e.g., the introduction of the pneumococcal conjugate vaccine and policies restricting antibiotic use applied in some countries, including France.…”
