Background Antibodies to histone have been associated in the adult literature with systemic lupus erythematosus(SLE) and drug induced lupus(DILE). Little data is available regarding the spectrum of pathology that antibodies to histone encompass in the pediatric population. Prior studies suggest an association with SLE, juvenile idiopathic arthritis(JIA), uveitis and linear scleroderma. Methods Patient charts were reviewed that contained positive anti-histone antibody testing during a consecutive three year period. Patient diagnosis along with the presence of: anti-histone antibody titer, ANA, and the presence of other autoantibodies to SSA, SSB, Sm, RNP, dsDNA and chromatin were obtained. The frequency of SLE, JIA and DILE was further investigated in specific subsets. Results 139 individual charts were reviewed containing 41 different diagnoses. The most common diagnosis was hypermobility arthralgia with 22 patients. The most frequent rheumatologic diagnosis was JIA(nonsystemic) with 19. 13 patients in this study were diagnosed with SLE and 2 with DILE. 18 patients had other autoantibody production, of these, 11 had SLE or DILE. Only one of 62 patients with a weak antihistone antibody titer(1.0-1.5) was diagnosed with SLE. When strong titers are present(> 2.5), the antihistone antibody test was associated with a greater than 50% incidence of an underlying rheumatologic disease and ten times higher incidence of SLE than a weak titer. In regards to the frequency of SLE, there was a statistically significant difference between weak and moderate titers and between weak and strong titers. Conclusion The presence of anti-histone antibody was observed in a variety of diagnoses in the pediatric population. Overall, the presence of anti-histone antibodies appears to have poor diagnostic utility for any specific condition. However, diagnostic utility for SLE does appear to improve with higher titers, when combined with other autoantibody positivity. Strength of titer did not appear to be a factor for JIA, but was the most frequently observed rheumatologic disease in this study.

Objective. To elucidate the nature of the antigen reactive with the "lupus erythematosus (LE) cell factor," the autoantibody involved in the LE cell phenomenon.Methods. Serum samples from systemic lupus erythematosus (SLE) patients who were positive for the LE cell phenomenon (LEc+) and SLE patients who were negative for the LE cell phenomenon (LEc-) were used to characterize the nuclear antigen bound by the LE cell factor, by immunoblotting and immunoprecipitation techniques.Results. All LEc+ sera, but none of the LEcsera, uniformly reacted with a double band of M W -30 kd in nuclear extracts. Depletion of nuclear protein extracts of antigens bound by pooled LEc-serum allowed precipitation of a low molecular weight protein by pooled LEc+ serum. This protein was able to block LE cell formation by LEc+ serum. Based on its reactivity with antihistone antibody and an electrophoretic mobility identical with that of precipitated and purified histone H1, this protein was identified as histone H1. Moreover, all LEc+ sera, but none of the LEc-sera, reacted with purified histone H1 by immunoblotting, whereas other histones were reactive with both types of sera. In addition, purified histone H1, but none of the other histones, strongly inhibited the induction of LE cells by LEc+ serum.Conclusion. Histone H1 represents the major antigenic component recognized by the LE cell factor. Thus, the LE cell phenomenon appears to be due primarily to anti-histone H1 reactivity.The lupus erythematosus (LE) cell phenomenon was recognized 50 years ago by Hargraves et a1 (1) constituted one of the first laboratory abnormalities ever found to be associated with systemic lupus erythematosus (SLE). It requires the presence of cell nuclei released from traumatized cells that usually have lost the normal chromatin pattern (2), antibodies originally termed LE cell factor (3), and complement (4). The ensuing engulfment of the opsonized nuclear material by polymorphonuclear cells leads to the classic appearance of the LE cell.After its original description, the elucidation of the mechanisms responsible for and related to LE cell formation became the center of interest for a generation of immunologists and rheumatologists. Kunkel, Holman, and Deicher observed that the LE cell factor was reactive with deoxyribonucleoproteins of the cell nucleus (5-7) and that it could be associated with pathologic events in SLE (8,9). Thus, the LE cell phenomenon was the first antinuclear antibody reactivity ever described and gave rise to a still-growing list of known autoantibodies to nuclear constituents. Meanwhile, several dozen nuclear antigens have been characterized and shown to react with autoantibodies from patients with SLE and various other rheumatic diseases (10-13).In spite of these advances, the autoantigen reactive with the LE cell factor has not been characterized, and the reactivities of the LE cell factor that are best known to date are the originally described and later confirmed ones with nucleosomes (5-7,14,15) which contain DNA and histones...

IntroductionThis article is part of a series on immunologic testing guidelines. The series introduction, published in this issue, outlines the full methodology for obtaining data, grading the literature, combining the information from multiple sources, and developing recommendations. Briefly, MEDLINE and Healthstar were searched using a variety of search terms, and all relevant available literature was reviewed. All articles were critically reviewed using published standards for studies of diagnostic tests. Test use was categorized as primarily diagnostic or prognostic (which also included monitoring). Information was extracted from each article to allow for calculation of a weighted average for sensitivity and specificity; likelihood ratios (LRs) were then derived from these values (positive LR ϭ sensitivity / [1-specificity]; negative LR ϭ [1-sensitivity] / specificity). Recommendations for use of tests were based on the LRs, where a test was considered to be "very useful" for a given disease if the weighted average positive LR was Ͼ 5 or negative LR was Ͻ 0.2. A test was considered "useful" if the weighted average positive LR was Ͼ 2 and Յ 5 or negative LR was Ͼ 0.2 and Յ 0.5. A test was considered "not useful" if the positive LR was Յ 2 or the negative LR was Ͼ 0.5. DefinitionAntinuclear antibodies (ANA) directed against a variety of nuclear antigens have been detected in the serum of patients with many rheumatic and nonrheumatic diseases, as well as in patients with no definable clinical syndrome. These antibodies can be detected using a variety of substrates and staining techniques as described below and are directed against many different nuclear antigens.
