Antibody Responses and Reactogenicity of a Heterologous, Full-Dose Messenger RNA-1273 Booster in Heavily SARS-CoV-2–Exposed CoronaVac-Vaccinated Health-Care Workers in Indonesia: A Real-World Observational Study

Abstract: Real-world data on heterologous boosting with messenger RNA (mRNA)-1273 (Moderna) after inactivated COVID-19 vaccination are limited. We report mRNA-1273 boosting in heavily SARS-CoV-2–exposed Indonesian health-care workers who received a two-dose CoronaVac 6 months prior. Between August and November 2021, we measured SARS-CoV-2 spike-specific IgG binding antibody (Bab) titers in all 304 participants, and neutralizing antibody titers in a random subset of 71 participants, on stored paired serum samples taken b… Show more

“…Thereby, it is recommended and safe to administer a heterologous booster vaccine, especially using mRNA vaccines, such as Moderna and Pfizer ( Goh et al, 2022 ). Similar to this study, investigations on Indonesian healthcare workers receiving two-dose CoronaVac and one dose of Moderna booster also showed 100% anti-SARS-CoV-2 antibodies positivity ( Cucunawangsih et al, 2022 ; Hidayat et al, 2022 ; Sinto et al, 2023 ).…”

“…Nonetheless, our study suggests that a higher mortality rate was associated with a lower number of nurses per population. In Indonesia, the lack of healthcare workers, combined with the high number of infections [ 40 ] and deaths among health care workers and doctors [ 41 ], especially during the early phase of the epidemic, had weakened the health system resilience. A recent review of Indonesian healthcare system suggested that the currently available medical staffs are insufficient to deal with the pandemic, and that the referral system in Indonesia has been hampered by a shortage of specialists and poorly equipped referral facilities, as well as weak coordination, even before the pandemic [ 42 ].…”

“…Several additional factors, which our analysis was not able to capture, may have further exacerbated the pandemic impacts on TB case finding and diagnostic and treatment services. First, high rates of infection 32 as well as deaths 33 among frontline health care workers resulted in substantial absenteeism and task-shifting, especially during the early pandemic. Notably, a recent report estimated that Indonesian healthcare workers had a five times higher chance of death from COVID-19 than the general population.…”
