The development and use of biochemical markers of cardiac damage is in a dynamic state. Creatine kinase (CK), aspartate aminotransferase (AST) and creatine kinase muscle± brain (CK±MB) remain in widespread use, but cardiac-speci®c isoforms of troponin T (cTnT) and troponin I (cTnI) have emerged as sensitive and speci®c indicators of myocardial infarction which are also used for risk strati®cation of patients with acute coronary syndrome. 1 The diagnostic performance of the troponins has been widely evaluated. It is important, however, that other factors, such as analytical performance, interpretation of results, availability out of hours and in vitro stability are considered before any new test is brought into routine use.Most laboratories store routine samples for up to 14 days at 48C. Retrospective analysis of cardiac markers is occasionally requested if the diagnosis of myocardial injury is not initially apparent or if the clinical presentation changes. Published and manufacturers' data on long-term in vitro stability of cardiac markers are limited. This study examines the stability of ®ve cardiac markers, CK, AST, CK±MB, cTnT and cTnI at 48C over 14 days.

Measurement of Cardiac Troponins

The cardiac troponins form part of the regulatory mechanism for muscle contraction. Specific cardiac isoforms of cardiac troponin T and cardiac troponin I exist and commercially available immunoassay systems have been developed for their measurement. A large number of clinical and analytical studies have been performed and the measurement of cardiac troponins is now considered the 'gold standard' biochemical test for diagnosis of myocardial damage. There have been advances in understanding the development and structure of troponins and their degradation following myocardial cell necrosis. This has contributed to the understanding of the problems with current assays. Greater clinical use has also highlighted areas of analytical and clinical confusion. The assays are reviewed based on manufacturers' information, current published material as well as the authors' in-house experience.

Cardiac troponin T and I (cTnT, cTnI) are sensitive biochemical markers of myocardial cell necrosis and have been adopted as the gold standard tests for acute myocardial infarction. Subtle elevations in cTn above the detection limits of the currently available commercial assays confers poor prognosis. These markers are superior to classical enzyme markers of necrosis due to their cardiospecificity. The diagnosis of drug-induced cardiac toxicity using the classical enzymes is problematic due to the high elevations of these markers in skeletal muscle necrosis. cTnT and cTnI are now being adopted as sensitive biomarkers of drug-induced cardiac toxicity.
