Anticonvulsant‐Induced Status Epilepticus in Lennox‐Gastaut Syndrome

Abstract: A 22-year-old woman with the Lennox-Gastaut syndrome developed serial apnoeic-tonic seizures with concomitant bursts of repetitive spikes at 16-20/sec, after receiving therapeutic doses of chlormethiazole, clonazepam, and diazepam for the treatment of serial tonic-clonic generalized seizures. There was a direct relationship between dose of chlormethiazole and frequency of apnoeic-tonic seizures. Since these tonic seizures have been noted during natural sleep and after sleep induced by various sedative drugs in… Show more

“…De novo appearance of myoclonic jerks was described in several reports on children or young adults with cryptogenic or symptomatic partial epilepsy treated with addon VGB (37,46,47). Only in a minority of these patients was the "epileptic" origin of myoclonus demonstrated.…”

“…If this hypothesis is well founded, response to treatment is likely to be determined more by the morphology of the EEG waveform than by the partial or generalized nature of the idiopathic epilepsy. A preliminary comparative analysis of the rolandic discharge morphology between patients with BRE accompanied only by typical facial (32)(33)(34)(35)(36)(37)(38). In most of these patients, BDZs had been administered for treatment of absence status.…”

“…[18][19][20][21][22] Benzodiazepinler GABA-A reseptörleri üzerinden etki ederler, GABA-B reseptörleri üzerinden etkileri yoktur o yüzden benzodiazepinlere bağlı LGS'de tonik status epileptikusun ortaya çıkması, yukarıda tartışılan talamik mekanizmalarla tam olarak açıklanamamaktadır. [4] Altta yatan ciddi beyin patolojisi [4] veya benzodiazepinlere bağlı somnolansın [23] artmış nöbet aktivitesine yol açtığı düşünülmektedir.…”

“…More interestingly, increased seizure activity can also occur when antiepileptic drugs are used appropriately. Tassinari and colleagues (1972) and, more recently, Bittencourt and Richens (1981) reported a paradoxical effect of intravenous benzodiazepines, producing status epilepticus in children with the Lennox-Gastaut syndrome. According to the latter authors, the decreased level of consciousness induced by benzodiazepines and other CNS depressants could be the cause of this paradoxical effect.…”
