Monoamine oxidase (MAO) inhibitors are frequently used for multidrug-resistant major depression, which is emerging as an epidemic in the modern era. Anesthesia during chronic use of MAO inhibitors is a matter of debate because of increased risk of drug interactions with various anesthetic drugs. Cardiac disorders contribute to perioperative and postoperative complications. Recent studies illustrate the safety of anesthesia without discontinuation of MAO inhibitors if sympathetic homeostasis is maintained and known drug interactions are avoided. In this case study, a 72-year-old male psychiatric patient on permanent treatment with tranylcypromine (30 mg/ day) was admitted for bipolar hemiarthroplasty. After complete aseptic precautions, spinal anesthesia was achieved by 12.5 mg 0.5% heavy bupivacaine and 30 µg clonidine intrathecally. The anesthetic effect was adequate, but surgery was not completed timely and the effect of spinal anesthesia was weaned off, so general anesthesia was given and surgery was completed. There was no perioperative or postoperative complication. In conclusion, general or regional anesthesia for noncardiac surgery without discontinuation of MAO inhibitors may be safe after careful preoperative evaluation of the patient.

Oxidative stress can be generated at several sites within the mitochondria. Among these, monoamine oxidases (MAO) have been described as a prominent source. MAO are mitochondrial flavoenzymes responsible for the oxidative deamination of catecholamines, serotonin and biogenic amines, and during this process they generate H2O2 and aldehyde intermediates. The role of MAO in cardiovascular pathophysiology has only recently gathered some attention since it has been demonstrated that both H2O2 and aldehydes may target mitochondrial function and consequently affect function and viability of the myocardium. In the present review, we will discuss the role of MAO in catecholamine and serotonin clearance and cycling in relation to cardiac structure and function. The relevant contribution of each MAO isoform (MAO-A or -B) will be discussed in relation to mitochondrial dysfunction and myocardial injury. Finally, we will examine both beneficial effects of their pharmacological or genetic inhibition along with potential adverse effects observed at baseline in MAO knockout mice, as well as the deleterious effects following their over-expression specifically at cardiomyocyte level.

Zusammenfassung Hintergrund Bei der Tako-Tsubo-Kardiomyopathie (TTK) handelt es sich um eine akute reversible Erkrankung des Herzmuskels. Sie wird ursächlich mit einem Überschuss an Adrenalin, Noradrenalin und Dopamin in Zusammenhang gebracht. Als bekannte Risikofaktoren gelten psychischer und physischer Stress. Es existieren Fallberichte über das Auftreten von TTK nach Krampfanfällen sowie EKT-Behandlungen. Methoden Es wird der Fall eines 63 Jahre alten Patienten vorgestellt, bei dem es im Verlauf einer zweiten EKT-Serie unter Etomidat-Narkose und laufender antidepressiver Medikation mit Tranylcypromin zu einer TTK kam. Mögliche pathophysiologische Zusammenhänge werden anhand der zur Verfügung stehenden Literatur diskutiert. In Anbetracht eines erhöhten Rezidivrisikos wird der Frage nachgegangen, ob die Wiederaufnahme der EKT-Behandlung nach Abklingen der TTK verantwortbar ist. Ergebnisse Depressive Störungen, EKT-Behandlungen mit konvulsiven Anfällen, der Einsatz von Etomidat als Narkotikum sowie eine Behandlung mit Tranylcypromin könnten eine theoretische Rolle als Risikofaktoren für die TTK spielen. Die durch Etomidat vermittelten Effekte könnten auf die substanzbedingte Störung der Kortisolsynthese und einer daraus resultierenden sympathomimetischen und pro-inflammatorischen Wirkung zurückgeführt werden. Es konnten 19 publizierte TTK-Fälle im Zusammenhang mit EKT gefunden werden. Einschließlich des hier vorgestellten Patienten wurde in 8 Fällen von einer erfolgreichen und komplikationslosen Wiederaufnahme der EKT-Behandlung berichtet. Diskussion Das Auftreten der TTK scheint letztlich multifaktoriell begründet zu sein. Die Konstellation unseres Falles sowie die analysierte medizinische Literatur erlauben keinen Rückschluss, dass einer der genannten Faktoren alleine für die Entstehung der TTK verantwortlich gemacht werden kann. Angesichts der theoretischen Überlegungen und Teilbefunde über die mögliche Rolle eines relativen Kortisol-Defizits für die TTK wären gezielte Untersuchungen wünschenswert. Die Wiederaufnahme der EKT-Behandlung nach einem TTK-Fall ist, unter Risikoabwägung und Berücksichtigung besonderer Vorsichtsmaßnahmen, nicht ausgeschlossen.
