Antineutrophil Cytoplasmic Antibodies in Children with Inflammatory Bowel Disease: Prevalence and Diagnostic Value

Abstract: Because of their sensitivity and specificity, ANCA may be helpful in the clinical assessment of patients with IBD, and especially those with UC. However, there is no link between the pressure of p-ANCA and the site of UC or its activity, so that it cannot be used to monitor medical treatment or surgical indications.

“…As noted above, antibodies against neutrophils have been found in many but not all UC patients (13,63,70,81). However, the titers of antineutrophil antibodies do not seem to correlate with disease activity in UC (98), and their biological significance in UC patients is unclear.…”

“…pANCA are markers of immunological disturbance but they do not injure the tissue directly. pANCAs were also found in healthy first-degree relatives of patients with inflammatory bowel diseases (Olives et al 1997). The transient pANCAs positivity can thus be considered to reflect temporarily injured subtle mechanisms maintaining the balance between tolerance and immunity to dietary and bacterial antigens in the large bowel of infants.…”

“…Until now, several markers have been reported to help in the differential diagnosis of UC and CD. They include p-ANCA, ASCA, anti-OmpC antibody, and anti-I2 antibody [13,14,22,23]. Recently, antibodies to four peptides selected from a phage display library were reported to be detected in 57% of CD patients [24].…”
