Antiretroviral therapy uptake and coverage in four HIV community cohort studies in sub‐Saharan Africa

Abstract: ObjectiveTo compare socio-demographic patterns in access to antiretroviral therapy (ART) across four community HIV cohort studies in Africa.MethodsData on voluntary counselling and testing and ART use among HIV-infected persons were analysed from Karonga (Malawi), Kisesa (Tanzania), Masaka (Uganda) and Manicaland (Zimbabwe), where free ART provision started between 2004 and 2007. ART coverage was compared across sites by calculating the proportion on ART among those estimated to need treatment, by age, sex and… Show more

“…This was probably due to the already high (89%) level of linkage to care in the standard‐of‐care (referral only) arm of that trial. In contrast, linkage to care in our standard‐of‐care arm was 33%, a figure consistent with that observed after routine referral in previous studies 20, 30, 31, 38, 39, 40.…”

“…On the basis of findings from HBHCT studies in which only referral was provided following HIV diagnosis 30, 31, 32, 33 and those in which counselling was provided after referral 6, 7, we assumed linkage of 35% in the control arm and that this would increase to 60% in the intervention arm. We aimed to have 80% power to detect this increase at a significance level of 5%.…”

“…This is of particular importance in sub-Saharan Africa where HIV prevalence rates are at their highest. Despite the roll-out of ART programmes in this region, ART coverage is still low [9]. Many HIV-infected patients seek healthcare with considerably more advanced immunodeficiency and may present with clinically different HHV-related diseases compared to individuals in resource-rich countries.…”

“…By mid-2008, ART uptake in the demographic surveillance area was estimated to be 58% of those eligible (12). Initial criteria for eligibility were WHO stage 3 or 4 disease or CD4 of <250 cells/mm 3 (after a brief period of <200).…”
