Antiretroviral treatment failure and associated factors among HIV patients on first-line antiretroviral treatment in Sekota, northeast Ethiopia

Abstract: Background Antiretroviral treatment has played a pivotal role in the reduction of HIV/AIDS-related morbidity and mortality. However, treatment options can be impaired by the development of antiretroviral treatment failure. Regular monitoring of the Human Immunodeficiency Virus treatment outcome via viral load tests is the key approach. There is a scarcity of information about HIV treatment failure and risk factors in the study area. Therefore, the study was aimed to assess antiretroviral treatment… Show more

“…Besides, antiretroviral therapy can prevent HIV from progressing, especially when a person starts taking it early. However, if the patients have treatment failure, HIV progressively kills the cells like CD4 T cells that help protect the body from infection, leading to complications [33][34][35].…”

“…Despite this, the need for life-long daily pill-taking can present substantial challenges that can be broadly grouped into three categories: adherence issues [4]; emotional issues, including stigmatization concerns and the daily reminder of HIV status [5]; and medical issues, such as difficulty swallowing, drug-food and drug-drug interactions, and pill burden [6]. These challenges can impact the likelihood of treatment failure, with poor adherence to antiretroviral therapy linked to the emergence of resistance and virologic failure [7][8][9][10]. Consequently, there is considerable interest across a range of patient demographics in long-acting therapies, in particular, injectable long-acting therapies, which may help to alleviate some of the challenges associated with daily oral therapy [11][12][13][14][15].…”

“…Increased patient satisfaction has been shown to improve adherence [23], which contributes to the maintenance of long-term HIV suppression as well as preventing drug resistance, slowing disease progression, and reducing mortality [9,10,[24][25][26]. PROs can aid treatment optimization as they provide key insights into quality-of-life issues that have the potential to influence adherence to life-long treatment.…”

“…Since taking HIV medicines as prescribed can help keep the viral suppression and your CD4 cell count high [ 33 , 34 ]. Moreover, treatment failure worsens the risk of mortality of the patient as a result of ARV combination and treatment duration, which implies the antiretroviral drugs are no longer able to suppress the virus or prevent the deterioration of your immune system [ 35 , 36 ].…”
