Apical sparing in tako‐tsubo cardiomyopathy

Abstract: The apical sparing variant of TTC is common, accounting for 40% of cases. While the patients are younger and more likely premenopausal, there are no other distinguishing features between the classical and the variant form.

“…The syndrome was first described in the Japanese population in 1991 and was given the name 'Tako-Tsubo' after the round bottomed and narrow necked Japanese fishing pot that resembles the characteristic shape of the left ventricle during systole in this condition [3]. Since then there have been several case series from North America [4,5,6,7,8], Europe [9,10,11], Asia [12,13,14] and Australia [15]. TTC has been recognized as a primary acquired cardiomyopathy in the American Heart Association scientific statement on the classification of cardiomyopathies [16].…”

“…Therefore, the state of oestrogen deficiency in the post-menopausal period may act to pre-dispose woman to Tako-tsubo syndrome, by removing an important means to modulate sympathetic outflow and microcirculatory resistance. The suggestion that oestrogen desensitises the heart to catecholamines and then creates a state of rebound hypersensitivity in post-menopausal women, is not supported by findings of Abdulla et al in a study showing the occurrence of Tako-tsubo syndrome in pre-menopausal women [43]. Whilst oestrogen deficiency may play a contributory role, it is unlikely to represent a mechanism for Tako-tsubo syndrome in its own right.…”

“…Jüngst wurde auch eine neue Variante des TTS mit Aussparung der Herzspitze beschrieben (Abbildung 2). Die Klinik und der zeitlicher Verlauf sind ähnlich wie bei der klassischen Form von Takotsubo [44].…”

“…Da diese Befunde unspezifisch sind, spielt die Myokardbiopsie beim typischen Bild eines TTS weder für die Diagnose noch für die Therapie eine Rolle [44,46].…”
