Appendiceal mucocele—A rare case report

Abstract: Highlights A Mucocele appendix with wide base communicating with caecum is a rare presentation. Presentation is same as acute appendicitis. Pre-operative diagnosis even with the help of USG & CT is really a challenging issue. Diagnosis of wide base mucocele appendix (preoperatively/intraoperatively) changes whole course of surgical management.

“…Histologically, appendiceal mucocele can be categorized into 4 causes: retention cyst, mucosal hyperplasia, LAMN or mucinous cystadenoma, and mucinous cystadenocarcinoma [ 4 - 6 , 9 ]. More than half of mucinous tumors (52% to 58%) are considered malignant tumors of the appendix [ 5 , 6 ]. Spontaneous or iatrogenic rupture of appendiceal mucocele may lead to dissemination of epithelial cells that produce mucins into the peritoneal cavity.…”

“…Without a twisting event, it is usually asymptomatic and often diagnosed incidentally. Perforation of an appendiceal mucocele may lead to catastrophic results, such as pseudomyxoma peritonei (PMP), depending on the histological finding of mucocele [ 4 , 5 ]. This is why it is greatly important to prevent rupture of the mucocele during surgery.…”

“…1 2 The incidence is relatively rare at 0.2%-0.3% in patients who had appendectomy, and cases preoperatively diagnosed in early stage are known to be very rare. 3 Appendicel mucocele may mimic other adnexal lesions such as tubo-ovarian abscess, hydrosalpinx and ovarian mucinous tumour. 4 5 Approximately 25% of cases of mucocele of the appendix are symptomless and identified by chance.…”

“…Due to the absence of characteristic clinical symptoms and some of which may be manifestations of abdominal internal hernia, it needs comprehensive examinations to diagnose ( 8 ). Well, regarding to abdominal internal hernia, there are characteristic signs of CT scan, so generally it can be confirm diagnosed by the CT scan ( 9 ). Herein we present a case of appendiceal mucinous cystadenoma presenting as abdominal internal hernia to provide much more information for clinical diagnosis and therapy.…”
