Appendicitis as a rare cause of mechanical small-bowel obstruction: A literature review of case reports

Abstract: HighlightsAppendicitis as a cause of small-bowel obstruction is an extremely rare. Our study confirmed that this complication is difficult to diagnose preoperatively.A conclusive diagnosis typically has to wait until visualization during the surgery.Surgical management varies from appendectomy up to right-sided hemicolectomy.

“…With closed loop obstruction, the inflamed appendix wraps around the last loop of the ileum (Fig. 5d ); the inflаmed tip of the аppendix аdheres to the posterior peritoneum or the root of the аppendix [ 2 , 12 , 13 ], or herniation of intestine occurs through a gap formed by the appendix tip being attached to its base [ 14 ], and this might lead to necrosis [ 7 , 8 , 15 ]. The mechanism and morphology of bowel obstruction relate to the position, mobility, and length of the appendix; an inflamed appendix might adhere to the other surrounding structures, such as the ileum, the mesentery of the ileum, the caecum, and/or the colon, the posterior peritoneum, or right tubo-ovarian complex [ 4 , 15 ].…”

“…In the case of SBO caused by acute appendicitis, CT is an additional examination helpful for diagnosis and prognosis [ 12 , 15 ], with enhancement of the dilated small bowel wall and intra-abdominal fluid. However, the role of CT in detecting appendicitis is also case-by-case; often, it is not found due to the inflamed appendix being wrapped around the ileum [ 11 , 12 , 13 , 15 ]. It may be seen in enlarged purulent appendicitis [ 3 ], as in our case.…”

“…When the appendix causes obstruction, it mostly occurs as a result of complication of the acute appendicitis in the form of ileus (secondary to peri-appendiceal inflammation or as part of generalized peritonitis) or trapped in an appendiceal mass [6]. The appendix can also cause obstruction by forming a knot with the ileum(AIK) [5].…”

“…But, all forms of intestinal knots are extremely rare during pregnancy. In AIK the distal ileum is entrapped by knot of the appendix which encircles the distal ileum resulting closed-loop obstruction [1, 3–5]. The vermiform appendix or an inflamed appendix as a cause of intestinal obstruction in pregnancy is exceedingly rare that we didn’t find a report in literature.…”

“…In a literature review by Ondrej and Jiri,4 only 17 cases from the period of 1963 to 2015 were found in which appendix was obstructing by knotting around the bowel loop. Out of these, seven patients were treated by simple appendectomy as there was no gangrene.…”
