Appendix vermiformis as a left pyelo-ureteral substitute in a 6-month-old girl with solitary kidney

Abstract: Extensive ureteral loss in early childhood is a rare but dramatic event. We present the case of a 6-month-old girl with a iatrogenic extensive pyelo-ureteral loss and solitary kidney. She successfully underwent left ureteral substitution using the appendix vermiformis. Left ureteral reconstruction using the appendix vermiformis is a feasible procedure even in very small children.

“…An appendiceal graft is more suitable for right-sided ureteral reconstruction due to its inherent ipsilateral location. The use of an appendiceal graft for left-sided ureteral reconstruction has been reported but it is less common due to its higher complication rate [ 6 , 8 ]. The long-term success of appendiceal ureteral replacement in terms of satisfactory transport of urine and maintaining good renal function, has been documented [ 9 ].…”

“…Initially a whole segment was used as a substitute but due to its drawbacks of lumen discrepancy, chances of reflux, excessive mucus formation, infections, loss of larger length of bowel and metabolic disturbances; now creating a tube from smaller bowel segment using a young Monti's principle is preferred.
