日臨外会誌 80(4) ,641-651,2019 DepartmentofThoracicSurgicalOncology,theCancerInstituteHospital,JapaneseFoundationforCancerResearch Althoughsurgicaltherapyhaswidelybeenperformedformetastaticlungtumors,anyguidelinesfor thesurgicalindicationsandproceduresarestillabsent.Asforsurgicaltherapiesformetastaticlungtumors,theissuestobediscussedinaccordancewithrealclinicalsettingsarelargelyclassifiedinto:1)criteriaforsurgicalindications;2)operativeproceduresincludingrangeoflungresection(wedgeresection, lobectomy,orsegmentectomy),thesignificanceofregionallymphnodedissection,andoperativeapproach (thoracoscopicoropenapproach);3)therapeuticguidelinesbyeachprimarylesion;and4)theroleof thesurgicaltherapyintheeraofmultidisciplinarytherapy.Thisreviewarticledealswithconsiderations basedonourexperiencesinourhospitalintermsof1)thehistoryofsurgicaltherapyformetastaticlung tumorsaswellastimecoursefromthehistoryuntilwehavereachedthecurrentsurgicalindicationcriteria, and those in our hospital;2) changes and current status of operative procedures in our hospital, a comparison with those described in the literature, and consideration of operative procedures used for wedgeresection;3)changesinoutcomesofsurgicalresectionbyeachprimarylesionandbyperiod;and 4)theroleandfurtherperspectivesofsurgicaltherapyintheeraofmultidisciplinarytherapy.
