Applied Anatomy of the Genital Branch of the Genitofemoral Nerve in Open Inguinal Herniorrhaphy

Abstract: To avoid damage to the genital branch of the genitofemoral nerve, suturing should be done over the dorsal aspect of the internal ring. The cremaster should be incised longitudinal rather than transversely and the genital branch of the genitofemoral nerve should be identified before repair of the posterior wall.

“…The courses of the ilioinguinal nerve and the GFN are very variable, and this variation should be taken into account in the examination and treatment of chronic groin pain. 1,14,17,19 In conclusion, central motor fibers to the CM motor neurons exist and a representation area for the CM may exist in the motor cortex. The GFN motor conduction time to the CM may have some clinical implications.…”

“…Its genital branch follows the psoas muscle and penetrates the internal inguinal ring with the spermatic cord, supplying motor fibers to the CM and sensation to the scrotum and upper thigh. 1,11,14,[17][18][19] The CM is a striated muscle but, for the most part, its voluntary control is not required. 10 Current knowledge of the physiological control of the CM is mainly restricted to the evaluation of the CM reflex.…”

“…Looking at the literature, only two anatomical studies have investigated the presence and course of the three inguinal nerves together [55,64], while others have investigated the presence and course of only one or two inguinal nerves [8,9,19,27,39,42,47,50,51,53,62].…”

“…The nerves emerge from the lateral border of the psoas major muscle and run subperitoneally in front of quadratus lumborum muscle before piercing the transverse abdominis (TA) muscle above the iliac crest. 75 The IH nerve runs downward and forward and pierces the internal oblique (IO) muscle above the anterior superior iliac spine (ASIS). It then travels between the IO muscle and the EO muscle.…”
