Applying Lean Six Sigma Methodologies to Improve Efficiency, Timeliness of Care, and Quality of Care in an Internal Medicine Residency Clinic

Abstract: This study shows that incremental changes in workflow processes in a residency clinic can have a significant impact on practice efficiency and adherence to scheduled visits for preventive health care and chronic disease management. This project used a structured "Plan-Do-Study-Act" approach.

“…We believe that a further specification can be declared within the remaining group, identifying two distinct sub-techniques. In one approach, researchers interpret and model qualitative data onto theoretical activity diagrams a-priori[45], and then enrich the abstracted workflow with time-data[46] by having observers record the duration of expected ordered milestones[47]. We named this approach “milestones timing”, which is the most similar to the original method described by Taylor.…”

“…7 Patient experiences may depend in part on the primary care facility at which care is received. Because of the competing demands of education and patient care, it often has been noted that busy teaching sites in particular have limited access 8,9 and long wait times 8,10,11 and often run inefficiently 8,11,12 because of a high volume of residents in training, their own competing priorities, 12,13 and the overall complexity of the teaching environment, which may be particularly true in the poor, underserved communities of the Bronx. However, residency programs are also centers of expertise.…”
