Are pelvic adhesions associated with pain, physical, emotional and functional characteristics of women presenting with chronic pelvic pain? A cluster analysis

Abstract: BackgroundChronic pelvic pain is a debilitating condition. It is unknown if there is a clinical phenotype for adhesive disorders. This study aimed to determine if the presence or absence, nature, severity and extent of adhesions correlated with demographic and patient reported clinical characteristics of women presenting with CPP.MethodsWomen undergoing a laparoscopy for the investigation of chronic pelvic pain were recruited prospectively; their pain and phenotypic characteristics were entered into a hierarch… Show more

“…In a study of 90 women who underwent laparoscopic salpingo-ovariolysis, 40.2% developed moderate to severe adhesion reformation identified during early second look laparoscopy ( Alborzi et al , 2003 ). Whilst no correlation has been identified between self-reported pain, physical or emotional scores with the presence or absence of pelvic adhesions identified during diagnostic laparoscopy ( Cheong et al , 2018 ), adhesions are associated with increased risk of infertility ( Vrijland et al , 2003 ), which would be counterproductive in a procedure intended to preserve and restore reproductive potential.…”

“…CPPS, which is defined as intermittent or continuous pain in the lower abdomen or pelvis lasting at least 6 months, can have negative cognitive, behavioral, sexual, and emotional consequences. CPP disorders are debilitating conditions which affect 38 in every 1,000 women (2), and account for 2-10% of all gynecological outpatient appointments, of which about 20% of cases undergo diagnostic laparoscopy (3), and 10-15% qualify for a hysterectomy (4). However, CPPS is still poorly understood and difficult to diagnose.…”
