Are Vocal Alterations Caused by Smoking in Reinke's Edema in Women Entirely Reversible After Microsurgery and Smoking Cessation?

Abstract: Abnormalities caused by smoking in Reinke's edema in women are not fully reversible with surgery and smoking cessation. One explanation would be the presence of possible structural alterations in fibroblasts caused by the toxicity of cigarette components, resulting in the uncontrolled production of fibrous matrix in the lamina propria, and preventing complete vocal recovery.

“…Likewise, vocal abuse and laryngo‐pharyngeal reflux (LPR) are often regarded as additional risk factors, albeit their separate impact remains unclear. Furthermore, a predominance in elderly females is reported, suggesting a contribution of the hormonal status 3–5 . Therapeutic recommendations are based on a cessation of these risk factors (smoking), a probatory treatment of LPR, and ultimately phonosurgery.…”

“…Furthermore, a predominance in elderly females is reported, suggesting a contribution of the hormonal status. [3][4][5] Therapeutic recommendations are based on a cessation of these risk factors (smoking), a probatory treatment of LPR, and ultimately phonosurgery. The latter leads mostly to an improvement of dysphonia; however, it cannot restore the healthy state.…”

“…Reinke's edema has an association with smoking, excessive vocal strain, and laryngo-pharyngeal reflux disease [2,3]. Its strong association with smoking is thought to be related to increased vascular permeability from the chemicals found in cigarettes [6][7][8]. Once a patient develops symptoms, the diagnosis is made by direct inspection of the vocal folds.…”

“…Estudos recentes mostraram que as anormalidades causadas pelo tabagismo no Edema de Reinke em mulheres não são totalmente reversíveis com a cirurgia e a cessação do tabagismo 14 . Quanto à concordância das análises vocal e laríngea, um estudo mostrou acorrer em cerca da metade dos casos estudos, somente 15 .…”
