Arginine depletion increases susceptibility to serious infections in preterm newborns

Abstract: Preterm newborns are highly susceptible to bacterial infections. This susceptibility is regarded as being due to immaturity of multiple pathways of the immune system. However, it is unclear whether a mechanism that unifies these different, suppressed pathways exists. Here, we argue that the immune vulnerability of the preterm neonate is critically related to arginine depletion. Arginine, a “conditionally essential” amino acid, is depleted in acute catabolic states, including sepsis. Its metabolism is highly co… Show more

“…In pediatric reports, a PCT result up to 2 ng/mL at 24 hours, or more than 4 ng/mL at 48 hours post-ST, was a good predictor of bacterial infection, with lower PCT values for localized infection and higher values in septic patients. Our higher values may be due to the increased susceptibility of newborns to generalized rather than localized infection, in contrast to older children and adults 26,27. Unlike other studies, in our sample, we did not find a subpopulation of patients with higher PCT values in the immediate postoperative period 1,21.…”

“…This susceptibility is attributed to the immaturity of multiple immune pathways that are related to arginine depletion 18 . CD71+ erythroid cells suppress immune responses via arginase-2 activity 11 .…”

“…Plasma arginine levels are likely to represent a balance between arginine intake, arginine synthesis, and the demands of protein synthesis and the multiple metabolic pathways for arginine utilization 53 . Arginine is a key substrate for nitric oxide synthesis and may explain the role of low plasma arginine levels in NEC, 44 , 45 pulmonary disease, 54 and impaired immune function 55 . The risk of NEC is reduced by arginine supplementation, 45 although more evidence is required 56 .…”
