Aripiprazole-induced priapism

Abstract: Priapism is a urologic emergency representing a true disorder of penile erection that persists beyond or is unrelated to sexual interest or stimulation. A variety of psychotropic drugs are known to produce priapism, albeit rarely, through their antagonistic action on alpha-1 adrenergic receptors. We report such a case of priapism induced by a single oral dose of 10 mg aripiprazole, a drug with the least affinity to adrenergic receptors among all atypical antipsychotics. Polymorphism of alpha-2A adrenergic rece… Show more

“…This is the first case report referring to a severe case of hepatic toxicity due to aripiprazole on usual doses. Despite the good tolerance and few side effects of aripiprazole, several case reports have revealed some rare, but relevant, side effects of the drug, including tardive dyskinesia [16], neuroleptic malignant syndrome [17], acute dystonia [18], parkinsonism [19], hypersexuality [20], priapism [21,22], neutropenia [23], skin rash [24], hypertension [25], hyperlipidemia [26], hypersensitivity pneumonitis [27], Raynaud’s phenomenon [26], hyponatremia [28], and sialorrhea [29].…”

“…Most reports of antipsychotic-induced priapism are of the ischemic kind. Antipsychotics are thought to induce priapism due to their alpha-1 adrenergic receptor blockade [ 4 , 6 ]. This allows parasympathetic activity to dominate and allows persistent vasodilation and subsequent erection of the penis.…”

“…There has been another reported case of aripiprazole-induced priapism, onset within seven hours of taking his first 10 mg dose [ 6 ]. This case was resolved with a single 2 mL injection of adrenaline and required no repeat treatment.…”

“…The most common cause of priapism is idiopathic, and other known causes include medications like antihypertensive drugs, selective serotonin reuptake inhibitors (SSRIs), phenothiazine, trazodone, and haloperidol; medical disorders like sickle cell disease (SCD); and illicit drugs like cocaine. The most common cause of priapism is medications in adults and SCD in children [1]. This is a case report of 30-year-old HIV-positive Caucasian male with history of schizoaffective disorder that developed painful penile erection after a single dose of sertraline 50 mg.…”
