Arrhythmias in Patients with Cardiac Amyloidosis: A Comprehensive Review on Clinical Management and Devices

Abstract: Cardiac amyloidosis (CA) is a rare but potentially life-threatening disease in which misfolded proteins accumulate in the cardiac wall tissue. Heart rhythm disorders in CA, including supraventricular arrhythmias, conduction system disturbances, or ventricular arrhythmias, play a major role in CA morbidity and mortality, and thus require supplementary management. Among them, AF is the most frequent arrhythmia during CA hospitalizations and is associated with significantly higher mortality, while ventricular arr… Show more

“…Border zones carry a heterogeneous structure that is able to organize itself into channels capable of conducting electrical stimuli; not by chance, the presence of channels inside fibrotic areas has already shown a strong association with sustained VAs [19] and the presence of fibrosis is one major mechanism responsible for arrhythmogenesis in patients with CA [20][21][22][23]. The presence of channels has already been identified as a useful tool currently under use for guiding ventricular tachycardia ablation [24], and recent data suggest that their reduction might represent a good indicator of a successful ablation procedure [25,26].…”

“…Although no data exist from randomized controlled trials with respect to the possible advantages of new oral anticoagulants versus VKAs in CA patients with AF, recent data suggest that DOACs can be used safely and effectively in CA patients [49].…”

“…It has been proposed also that drugs having a negative inotropic or chronotropic effect should be avoided, or at least used with caution at the minimum therapeutic dosage as rate or rhythm control; conversely amiodarone remains to be the best drug for rhythm control [ 10 , 26 , 27 ].…”
